【摘要】 目的:运用META分析评价乌头汤治疗类风湿关节炎(RA)的有效程度及安全程度。方法:采用Cochrane系统评价方法,通过计算机检索Cochrane Central Register of Controlled Trial、Embase数据库、中国知网资源总库中乌头汤治疗类风湿关节炎的随机对照试验,时间截止2017年3月1日,运用改良JADAD量表做证据质量评估;运用RevMan5.3.5软件对数据统计分析,包括异质性检验、meta-分析和敏感性分析。结果:①共检索到122篇乌头汤治疗类风湿关节炎的有关临床文献,最终纳入统计分析文献14篇,研究对象为1194人,运用改良JADAD量表评分,1篇为高质量文献,其余为低质量文献。②乌头汤或联合西药治疗类风湿关节炎与单用西药比较存在统计学差异(P<0.05);③在不良反应方面:乌头汤治疗组与对照组无统计学差异。结论:①单用乌头汤疗效优于单用西药;②乌头汤联合西药疗效优于单用西药;③乌头汤联合西药在治疗类风湿关节炎的效果、改善症状及降低相应的临床检验指标方面优于单用西药;④基于本次系统评价中显示出相关文献质量不高的情况和现状,今后乌头汤治疗类风湿关节炎的疗效及安全性研究需在中医辨证论治的基础上,严格按照随机双盲对照试验的步骤和要求,进行更大样本,更高质量,更多联合的临床研究,从而进一步准确对其开展系统评价。更多还原

【Abstract】 Objective:To evaluate the therapeutic efficacy and the safety of aconite decoction for treating rheumatoid arthritis(RA).Methods:The evaluation method of Cochrane system,Cochrane Central Register of to retrieve randomized controlled trials Controlled Trial,Embase database,China HowNet pool of Aconite Decoction in the treatment of rheumatoid arthritis by computer,the deadline of March 1,2017,using the improved JADAD scale to do the proof quality evaluation;analysis using RevMan5.3.5 software for statistical data,including heterogeneity test,meta-analysis and sensitivity analysis.Results:①122 pieces of Aconite Decoction in the treatment of rheumatoid arthritis related clinical literatures were retrieved,finally included in the statistical analysis of 14 articles,the research object is 1194 people,the evidence for the quality evaluation of the use of improved JAD AD scale,1 were of high quality literature and the rest were of low quality literature.②There were significant differences between aconite decoction or its combined with Western medicine and Western medicine alone in treatment of RA(P<0.05).③There was no significant difference in adverse reactions between the treatment group and the control group.Conclusion:①The efficiency of aconite decoction treating RA is better than Western medicine.②The efficiency of aconite decoction combined with Western medicine treating RA is better than Western medicine alone.③Aconite Decoction Combined with western medicine in the treatment of rheumatoid arthritis effect,improve symptoms and reduce the corresponding clinical examination index is better than single use of Western Medicine.?Research into the influence on the overall quality is low,heterogeneity and other factors based on Aconite Decoction in the treatment of rheumatoid arthritis in the efficacy and safety of TCM on the basis of strict accordance with the procedures and requirements of double blind randomized controlled trial,a larger sample,higher quality,more clinical research association thus,further to carry out its accurate evaluation system.更多还原
