预荷包缝合技术在腔外生长型胃黏膜下肿瘤内镜治疗中的应用

【摘要】 目的:探讨金属夹和尼龙绳联合行内镜下“预荷包缝合”技术在腔外生长型胃黏膜下肿瘤内镜治疗中的临床疗效。方法:回顾性分析2016年8月至10月在武汉大学人民医院消化内镜中心的27例腔外生长型胃黏膜下肿瘤行内镜下全层切除术的临床资料,根据术中是否进行预荷包缝合穿孔修补术,分为两组,一组为“预荷包缝合”技术完成,一组为常规内镜下全层切除术穿孔后行“荷包缝合”组。结果:预荷包缝合的15例病人中,胃窦3例,胃体6例和胃底6例;病变直径1.5 cm~4.0 cm,病变范围 1.5cm×1.5cm~4.0cm×4.0cm,平均大小 2.5cm×3.2cm;所有病人均顺利完成内镜下手术,操作时间为28.0~43.0(35.8±6.1)min;术后无明显并发症发生。常规方法的12例患者中,位于胃窦大弯侧2例、胃窦后壁1例、胃体大弯侧4例,胃体前壁1例及胃底4例。病灶直径1.3~3.7 cm,病变范围1.3 cm×1.5 cm~3.0 cm×3.7 cm,平均大小 2.4 cm×3.0 cm,手术时间为 45.6~68.8(48.8±5.3)min,术后均无明显并发症发生。结论:金属夹和尼龙绳联合行内镜下“预荷包缝合”技术在腔外生长型胃黏膜下肿瘤内镜治疗中的临床治疗效果可,值得推广。

【Abstract】 Objective:To investigate the clinical effect of the pre purse-string technique in the endoscopic therapy for stomach extra cavity growth type lesions.The pre purse-string technique is completed by the combination of metal clip and nylon rope.Methods:The clinical data of 27 patients with stomach extra cavity growth type lesions in the digestive endoscopy center of Renmin Hospital of Wuhan University from August 2016 to October were analyzed retrospectively.Results:There are 15 patients in pre purse-string group,3 cases gastric antrum,6 gastric body and 6 gastric fandus.The lesions ranged from 1.5cmx1.5cm~4.0cmx4.0cm with an average size of 2.5cm×3.2 cm.There are 12 cases in purse-string group,3 gastric antrum,5 gastric body and 4 gastric fundus,The lesions ranged from 1.3cm×l.5cm~3.0cmx3.7cm with an average size of 2.4cm×3.0cm.All patients were successfully completed endoscopic surgery,the operation time was 28.0-43.0(35.8±6.1)min in pre purse-string group,and 45.6~68.8(48.8±5.3)min in purse-string group.No obvious postoperative complications occurred.Conclusion:The clinical effect of "pre purse-string" technique in endoscopic treatment of patients with extra cavity growth gastric disease is well and worthy of promotion.
