PALB2在去势抵抗性前列腺癌中的表达及意义

【摘要】 目的探讨抑癌基因乳腺癌易感基因2(breast cancer susceptibility gene 2,BRCA2)定位协作因子(partner and localizer of BRCA2,PALB2)基因在去势抵抗性前列腺癌(castration-resistant prostate cancer,CRPC)组织中的表达及其与患者临床病理特征的关系。方法选取2015年1月—2017年6月天津医科大学第二医院收集的46例CRPC组织标本及其相关临床病理资料,包括前列腺腺癌36例,神经内分泌型前列腺癌(neuroendocrine prostate cancer,NEPC)10例,另取同期手术切除的20例良性前列腺增生(benign prostatic hyperplasia,BPH)组织标本作为对照。采用免疫组织化学法检测PALB2蛋白的表达情况,并分析其表达与CRPC患者临床病理特征的关系。结果PALB2蛋白主要表达在前列腺组织细胞胞质,PALB2蛋白在CRPC患者前列腺癌组织中的阳性表达率为80.4%(37/46),明显低于BPH患者的95.0%(19/20)(P=0.024);年龄、原发肿瘤局部情况、是否存在区域淋巴结转移和前列腺特异性标志物(prostate specific antigen,PSA)水平高低对PALB2的表达无明显影响(P值均>0.05),Gleason评分和病理类型影响PALB2的表达阳性率(P值均<0.05),Gleason评分>8者前列腺癌组织中PALB2蛋白的阳性表达率低于Gleason评分≤8者(P=0.024),NEPC患者前列腺癌组织中PALB2蛋白阳性表达率低于前列腺腺癌组织(P=0.004)。进一步的相关分析显示:PALB2的表达水平与腺癌患者Gleason评分呈负相关(r=-0.376,P=0.024),与病理类型也相关(r=-0.418,P=0.004)。结论PALB2在CRPC组织中表达阳性率降低,一定程度上可以反映CRPC的增殖与分化程度,参与肿瘤的发生发展。

【Abstract】 ObjectiveTo study the relationship of the expression level of partner and localizer of breast cancer susceptibility gene 2(BRCA2)(PALB2)in the castration-resistant prostate cancer(CRPC)tissues with its clinical and pathological features.MethodsForty-six CRPC specimens and its clinical and pathological factors including 36 samples from patients with prostate adenocarcinoma,10 samples from patients with neuroendocrine prostate cancer(NEPC)and 20 benign prostatic hyperplasia(BPH)specimens(as the control)were collected from January 2015 to June 2017 in the Second Hospital of Tianjin Medical University.The expression of PALB2 protein was detected by immunohistochemistry.The relationships of PALB2 expression with the clinical and pathological features of CRPC patients were analyzed.ResultsThe positive expression rate of PALB2 protein in CRPC tissues was 80.4%(37/46),which was significantly lower than that in BPH tissues [95.0%(19/20),P = 0.024].The positive expression rate of PALB2 protein was not significantly different in terms of age,primary tumor status,lymph node metastasis and prostate-specific antigen(PSA)level(all P > 0.05),but it was significantly different in terms of Gleason score and pathological type of CRPC(both P < 0.05).The expression level of PALB2 in patients with Gleason score >8 was lower than that in patients with Gleason score≤8(P = 0.026).Its expression level was also significantly lower in the NEPC than in the prostate adenocarcinoma tissue(P = 0.005).Further correlation analysis showed that PALB2 expression rate was negatively correlated to Gleason score(r =-0.376,P = 0.024)and pathological type(r =-0.418,P = 0.004).ConclusionThe positive rate of PALB2 expression is decreased in CRPC tissues,which can reflect the proliferation and differentiation of CRPC cells and may be correlated to the occurrence and progress of tumors.
