成人骨性Ⅲ类偏颌畸形颅面硬组织结构三维对称性分析

【摘要】 目的:利用锥体束CT(CBCT)技术研究骨性Ⅲ类下颌偏斜患者的颅面硬组织结构,探索不对称发生的部位与程度。方法:骨性Ⅲ类错(牙合)成人伴下颌偏斜患者34例作为偏斜组,不伴偏斜31例作为非偏斜组,采用CBCT扫描,对颅面硬组织22个标志点的不对称指数进行统计学分析。22个标志点分别位于六个分区:上颌骨区、下颌升支区、下颌体区、颧骨颧弓区、颞骨区、鼻骨区。结果:1.两组颏下点、上下颌磨牙及牙槽嵴点、下颌切牙及牙槽嵴点、下颌角点、喙突点的不对称指数存在统计学差异,均为偏斜组大于非偏斜组。2.下颌切牙及牙槽嵴点、下颌磨牙牙槽嵴点与颏下点偏移呈中度正相关;上颌磨牙及牙槽嵴点、下颌磨牙点、喙突点、上颌切牙点与颏下点偏移呈弱的正相关性。3.在31例非偏斜者中,有9例发生了颌骨的区域性不对称,其中仅上颌骨、仅下颌体、仅下颌支不对称者各3例,均占9.68%;34例偏斜患者样本中有10例(29.41%)患者发生了上下颌骨的不对称,其余24例(70.59%)为仅下颌骨的不对称。结论:1.与骨性Ⅲ类非偏斜者相比,偏斜患者存在下颌体、下颌升支、上颌牙槽部位的不对称。2.骨性Ⅲ类偏斜患者,下颌部位表现为骨性的不对称,上颌部位表现为牙及牙槽的代偿,下颌骨的不对称生长导致上颌骨发生继发性不对称。3.骨性Ⅲ类偏斜者颅底、颞骨、颧骨、鼻骨相关标志点的不对称性与骨性Ⅲ类非偏斜者相比无明显差异。
【Abstract】 Objective: We tried to study the craniofacial hard tissue structure of patients with skeletal Class III facial asymmetry using cone-beam computerized tomography(CBCT)technology,and to explore the site and degree of asymmetry.Methods: Thirty-four skeletal class III malocclusion in adult patients with mandibular deviation were included in the deviated group and thirty-one without deviation as the non-deviated group.The CBCT scan was used to analyze the asymmetry index of 22 landmarks in craniofacial hard tissues.Twenty-two landmarks were located in six subregions: the maxillary region,mandibular ramus region,mandibular body region,zygomatic bone and zygomatic arch region,temporal bone region,and nasal bone region.Results:1.There were statistically significant differences of the asymmetry index between the two groups and points focus on the menton,upper and lower molars and alveolar process,lower incisors and alveolar process,gonion,coracoid points,and the asymmetry index on the deviated group was larger than the non-deviated group.2.There is a moderate positive correlation between menton shift and asymmetry of lower incisors,alveolar process of lower incisors,lower molars alveolar process;There is a weak positive correlation between menton shift and asymmetry of upper molars,alveolar process of upper molars,lower molars,coracoid points,upper incisors.3.In 9 of 31 non-deviated patients,regional asymmetry of jaw occurred.Among them,3 cases(9.68%)showed only maxillary asymmetry,3 cases(9.68%)showed only mandibular body asymmetry,and 3 cases(9.68%)showed only mandibular ramus asymmetry.Among the 34 deviated patients,there were 10 cases(29.41%)of asymmetry of maxillary and mandibular,and the remaining 24 cases(70.59%)were asymmetry of the mandible only.Conclusion:1.Compared with skeletal class III non-deviated people,there is an asymmetry in the mandibular body,the mandibular ramus,and the maxillary alveolar area.2.In skeletal Class III deviated patients,the mandibular area is characterized by osseous asymmetry,and the maxillary site is represented by the compensation of teeth and alveolar ridges.Asymmetric growth of the mandible leads to secondary asymmetry in the maxillary.3.The asymmetry of the relevant landmarks of skull base,zygomatic bone,temporal bone,and nasal bone in skeletal class III deviants was not significantly different from that of skeletal class III non-deviated.
