对比乳腺X线及超声检查对乳腺钙化性病变的诊断价值
Comparative Study of Diagnostic Value in Mammography and Ultrasonography for Breast Calcification

【摘要】 目的:评价单独使用乳腺X线摄影、单独使用高频超声检查技术在诊断乳腺伴钙化性病变的意义,并探讨将二者联合使用对疾病的检出是否有意义。方法:收集我院经乳腺X线摄影检查和高频超声检查,有钙化存在,经病理证实的乳腺钙化性病变患者122例,其中24例不可触及到微小病变术前行二维钩丝定位。采用全数字化SIMENS乳腺机进行乳腺X线摄影检查,观察记录乳腺钙化病灶的大小,位置,数目,形态和分布类型征象,综合分析诊断病变良恶性。采用GE LOGIQ 9彩色多普勒超声检查仪进行超声检查,多角度侧动探头以显示恶性钙化,然后行彩色多普勒血流成像(Color doppler flow imaging,CDFI)观察病变周边和内部的血流情况,综合分析病变判断其良恶性。应用SPSS 19.0软件包进行统计方法及数据处理,计数资料用X2检验(如果不满足X2检验条件,采用Fisher’ S精确概率法),以P<0.05为差异具有统计学意义。结果:122例乳腺伴钙化疾病患者病理诊断结果:乳腺X线摄影对乳腺钙化性疾病诊断的敏感性、特异性、阳性预测值、阴性预测值、漏诊率、误诊率、准确性分别为 85.3%、70.2%、82.1%、75.0%、29.8%、14.7%、79.5%;高频超声对乳腺钙化性疾病诊断的敏感性、特异性、阳性预测值、阴性预测值、漏诊率、误诊率、准确性分别为78.7%、66.0%、78.7%、66.0%、34.0%、21.3%、73.8%;二者联合应用分别为 94.6%、93.6%、95.6%、91.7%、6.4%、5.3%、94.3%。二者联合应用诊断的准确性高于乳腺X线或超声单独应用(P<0.05)。结论:1.乳腺X线摄影通过钙化特征来鉴别乳腺良恶性疾病具有一定意义,通过彩色多普勒超声观察血流信号对诊断乳腺癌具有一定指导价值。2.乳腺X线摄影和高频超声的联合使用可以优势互补,对乳腺钙化性病变的诊断优于其单独诊断。3.二维钩丝定位可以提高乳腺微小病变手术的精确性。
【Abstract】 Objective:To evaluate the significance of the use of mammography or ultrasonography alone in the diagnosis of breast with calcification,and to explore whether the combination of the two can improve the efficiency of diagnosis.Methods:Through mammography and ultrasonography exmination,122 patients all have calcified lesions,who had not undergone biopsy,physiotherapy,surgery and Chemotherapy.These cases confirmed by pathology or postoperative puncture were collected.SIMENS full digital mammography,photography enlarged partial point pressure if necessary.The size,location,number,shape and distribution of breast lesions and Calcification were abserved and recorded.The results were analyzed.Using the GE LOGIQ 9 color Doppler ultrasonography,record the size of the lesion,edge,shape,internal echo,ratio.And then color Doppler flow imaging(CDFI)to observe the surrounding tumor and internal blood flow,comprehensive analysis of lesions to determine its Benign and malignant.SPSS 19.0 software package for statistical methods and data processing counting date with X2 test(if not meet the X2 test,using Fisher’s exact probability method)to P<0.05 for the difference was statistically significant,P<0.01 for the difference with statistically significant.Result:122 cases of breast with calcification disease pathological diagnosis:Mammography sensitivity calcified breast disease diagnosis,specificity,positive predictive value,negative predictive value,false positive rate,false negative rate,accuracy were 85.3%,70.2%,82.1%,75.0%,29.8%,14.7%,79.5%;The sensitivity,specificity,positive predictive value,negative predictive value,false positive rate,false negative rate,accuracy of ultrasonography were 78.7%,66.0%,78.7%,66.0%,34.0%,21.3%,73.8%.The combined using was 94.6%,93.6%,95.6%,91.7%,6.4%,5.3%,94.3%respectively.The combination of the two can improve the accuracy of breast calcification lesions(P<0.05).Conclusions:1.The calcification characteristics to identify benign and malignant breast disease in Mammography,which has a certain significance.Color Doppler flow imaging observation of blood flow signal diagnosis of breast cancer has a certain guiding value.2.If mammography and ultrasonography were combination,the diagnosis of breast calcification lesions is superior to its individual diagnosis.
