慢性间歇低氧对牙周炎大鼠血清及牙龈组织中IL-18、HIF-1α的影响

【摘要】 研究背景:阻塞性睡眠呼吸暂停低通气综合征(OSAHS)是一种潜在危害多组织器官,多学科交叉诊疗为特点的疾病。因其较高的发病率,人们健康日益面临严峻挑战,再加上其潜在危害大,所以成为当前的研究热点。慢性间歇低氧是OSAHS主要的病理特点。慢性间歇低氧通过机体内的氧化应激反应形成的系统性炎症,被认为与多种疾病相关。而牙周炎是口腔常见疾病。有流行病学调查表明:OSAHS与牙周炎的发生有显著的关系。但是OSAHS和牙周炎发病相关机制,目前尚不清楚。我们通过建立慢性间歇低氧下的牙周炎大鼠模型,检测慢性间歇低氧对IL-18、HIF-1α的影响,初步探讨两者的相关机制。方法:将32只SD大鼠分为4组,即A组:常氧对照组(常规喂养),B组:常氧牙周炎组(上颌双侧第二磨牙颈部结扎),C组间歇低氧组(置于模拟OSAHS的间歇低氧舱,每天8h),D组:间歇低氧牙周炎组(处理同B组+C组)。实验周期为8周。于实验结束后处死大鼠,对牙周检测各相关指标,用ELISA法测定4组大鼠牙龈及血清中IL-18和HIF-1α的浓度变化,并分析附着丧失与其的相关性。结果:1.建模8周后,AL的统计结果显示:B、C组的AL值要高于A组,差异有统计意义(P<0.05)。D组的AL值高于B、C组,差异有统计意义(P<0.05),以A组最低。BI的统计结果:A组与C组之间、B组与D组之间的差异无统计学意义(P>0.05);B组较于A组,D组较于C组,差异均有统计学意义(P<0.05)。2.在4组大鼠中,D组大鼠出现最严重的牙槽骨吸收,影像学显示根分叉暴露且伴有垂直型骨吸收。3.C组IL-18、HIF-1α浓度要显著高于A组,差异有统计学意义(P<0.05);D组IL-18、HIF-1α浓度要高于B组、C组,差异均有统计学意义(P<0.05)。4.D组大鼠AL与IL-18、HIF-1α浓度呈正相关(P<0.05)。结论:在本实验间歇低氧条件下,成功建立牙周炎大鼠模型。在慢性间歇低氧下,牙周附着丧失增加,并且与牙龈及血清中IL-18和HIF-1α的表达有关。
【Abstract】 Background:Obstructive sleep apnea-hypopnea syndrome is a high incidence, potentially dangerous diseases, which is characterized by multidisciplinary cross diagnosis and treatment.Because of its associated with many diseases, therefore become the research hot spot.Chronic intermittent hypoxia is the main pathological characteristics of OSAHS,chronic intermittent hypoxia by oxidative stress reactions in the body, to form the body of systemic inflammation. And periodontitis is an common disease. There are epidemiological investigation showed that OSAHS have significant relationship with the happen of periodontitis.At present there are not many research for OSAHS and perodontitis.We through the establishment of periodontitis rats model in the case of chronic intermittent hypoxia, detecting chronic intermittent hypoxia on IL- 18、HIF-1α,and analyse the possible mechanisms among OSAHS and periodontitis.Methods:32 SD rats were randomly divided into 4 groups, A: normoxia control group(Regular feeding); B: normoxia periodontitis group(ligating the bilateral maxillary second molar); C: intermittent hypoxia control group(under the CIH condition mimicking OSAHS, 8h per day); D: intermittent hypoxia periodontitis group(treatment with group B + C). Experimental period for 8 weeks. ALL SD rats were sacrificed in end of the experiment. The indexes of periodontal were detected. The concentration of IL –18, HIF-1α in periodontitis tissue and serums were measured using enzyme linked immunosorbent assay(ELISA), and analyse the correlation with perodontal index.Results:1.After 8 weeks,B group and C group than the A group compared to AL were significantly increased(P < 0.05); group than the B and C compared to AL were signifantly increased(P<0.05).2.D group is the most severe destruction of alveoar bone in 4 group.Radiograpgy demonstrated the exposure of root furcation and accompanied by vertical bone resorption.3.Compared with A group,the levels of IL-18、HIF-1α in B and C group increased(P< 0.05); Compared with D group, the levels of IL-18 、 HIF-1α in B and C group decreased(P<0.05).4.This study AL, IL-18 、 HIF-1α in D group comparisons between the positive correlation: AL and IL-18 compared positively correlated(P < 0.05); AL and HIF-1αcompared positively correlated(P<0.05).Conclusion:Under the condition of CIH in this experiment, periodontitis rats model were successful established; Chronic intermittent hypoxia can lead AL increased in rats, which is correlated with the expression of IL-18 and HIF-1α in serum and gingival tissues.
