【摘要】 研究目的：本研究旨在循证的基础上,建立符合我国国情的NICU护理质量敏感性指标,为NICU护理质量评价与监测提供依据。研究方法：本研究制定详尽的检索策略,采用美国霍普金斯证据等级与质量评价方法对检索到的相关文献进行等级与质量评定,总结出NICU护理质量敏感性指标、计算公式及资料收集方法,通过专家小组讨论初步拟定适合中国国情的NICU护理质量敏感性指标。采用Delphi咨询法,按照Likerts5级评分法,对初步形成的指标条目的重要性、计算公式的合理性、收集资料方法的可操作性进行评价。由6个省份的18家三级医院的NICU专家进行函询论证,确立符合中国国情的NICU护理质量敏感性指标及其计算公式与资料收集方法。运用专家积极系数、权威系数、变异系数、协调系数等指标,对专家函询的可靠性和代表性进行检验。研究结果：构建了疼痛处理率、噪音发生率、ROP发生率等11项NICU护理质量敏感性指标,包含了指标内涵、计算公式、分子分母释义、资料收集方法。2轮专家函询的积极系数分别为95.12%和97.44%(>60%),专家的权威系数分别为0.86和0.87(≥0.7)；2轮专家函询后各项指标重要性的变异系数为0.00-0.23,计算公式合理性的变异系数为0.03-0.27,在收集资料方法可操作性方面,除疼痛处理率和噪音发生率变异系数分别为0.34和0.30外,均小于0.3。研究结论：构建了一套符合中国国情的11项NICU护理质量敏感性指标,确立了各指标的内涵、计算公式及资料收集方法。专家函询的积极系数、权威系数、变异系数等结果证明专家函询代表性和可信度高。更多还原

【Abstract】 Objective:To set the NICU nursing quality sensitivity index accord with our national conditions, so as to provide reference for NICU nursing evaluation and monitoring.Methods:Based on the rigorous retrieval strategy, using the Johns Hopkins level of evidence and quality evaluation method to analyze literatures, the nursing quality sensitivity indicators, their calculation formulas and data collection methods were summed up. Using Delphi method, according to the 5 Likert scale method, the initial formation of the importance of index entries, the rationality of the calculation formula and the operability of data collection methods were evaluated. By consultation of NICU experts from 18 three-grade hospital of 6 provinces, the sensitive nursing quality indicators with calculation formulas and data collection methods were established, which are suitable for China’s national condition. The enthusiasm coefficient of experts, the authority coefficient of experts, the coefficients of variation and the harmonization coefficient were used to examine the reliability and representation of expert consultation.Results:11 NICU nursing quality sensitivity index was set such as pain treatment rate, noise incidence rate, ROP incidence rate and so on, of which target connotation, calculation formula, document collection methods is included. For the two rounds of questionnaire,the valid rate of which is 95.12% and 97.44%(>60%), the authority scores is 0.86 and 0.87 (≥0.7), after the two rounds of questionnaire, coefficient of variation of the importance of index is 0.00-0.23, coefficient of variation of rationality of calculation is 0.03-0.27. For the operability of document collection, all the coefficient of variation is less than 0.3 except pain treatment rate which is 0.34, noise incidence rate which is 0.30.Conclusions:11 NICU nursing quality sensitivity index accord with our national conditions was established. The index connotation, the ways of calculation and document collection was set. The reliability of expert consultation showed that the expert consultations had good reliability and representation.更多还原
