脑卒中后肩痛患者肩部红外热成像观察

【摘要】 目的观察脑卒中患者及伴有肩手综合征、痉挛和肩关节半脱位的脑卒中患者的肩部皮肤温度变化特点,并探讨红外热成像检查是否可作为脑卒中后肩痛的客观评价指标。方法选取86例脑卒中患者,使用数位式红外线热影像测温系统对脑卒中患者,伴有肩手综合征、痉挛和肩关节半脱位的脑卒中患者,以及脑卒中后肩痛患者进行肩部红外热成像观察,摄取正前、正后及侧面的热成像图,记录感兴趣区健、患侧肩部的平均温度并计算其相应的温差。结果70例(81.4%)患者患侧肩部较健侧温度低,健患侧比较差异有统计学意义(P<0.05)。肩手综合征患者组两侧肩部皮肤温差与非肩手综合征患者比较,差异有统计学意义(P<0.05);痉挛组患者与非痉挛组以及肩关节半脱位与非肩关节半脱位患者比较,差异无统计学意义(P>0.05)。脑卒中肩痛组与非肩痛患者比较,差异无统计学意义(P>0.05)。结论红外热成像检查能客观反映脑卒中患者肩部皮肤温度变化特点,大部分患者的患侧肩部皮肤温度较健侧低。合并肩手综合征的患者两侧皮肤温度不对称现象更明显,痉挛和肩关节半脱位不会明显影响皮肤温度。脑卒中后肩痛患者与非肩痛患者的红外热成像表现无明显差异。
【Abstract】 Objective To observe shoulder skin temperature characteristics in hemiplegic patients and in patients with shoulder-hand syndrome, spasticity, and shoulder subluxation, and to explore whether the infrared thermal imaging can be used as an objective evaluation index for post-stroke shoulder pain.Methods Select86cases of stroke patients, using digital infrared thermal imaging temperature measurement system on hemiplegic patients and patients with shoulder-hand syndrome, spasticity and shoulder subluxation to observe their shoulder skin temperature characteristics. And compare skin temperature differences of shoulder between post-stroke shoulder pain patients and without post-stroke shoulder pain patients.Results86cases of stroke patients,73cases (84.9%) patients with abnormal asymmetric thermal image,70cases (81.4%) patients with the affected shoulder lower than the contralateral shoulder, In patients with shoulder-hand syndrome compared with patients without shoulder-hand syndrome, the difference has a significant meaning (P<0.05);spasm and non-spasm patients as well as shoulder subluxation and without shoulder subluxation patients, the differences are both no statistically significant (P>0.05). Comparison of shoulder pain and non-shoulder pain patients, differences has no significantly meaning (P>0.05).Conclusion Infra-red thermal imaging temperature measurement system can objectively reflect shoulder skin temperature changes in stroke patients.The skin temperature in patients with shoulder-hand syndrome is more asymmetric, but spasticity and shoulder subluxation can not significantly affect the results. Patients with subjective pain cannot be reflected by changes in skin temperature.
