丹参酮IIA对急性心肌缺血室性心律失常的影响
Effects of TanshinoneⅡA on Ventricular Arrhythmias Induced by Acute Myocardial Ischemia

【摘要】 目的:观察丹参酮ⅡA在兔急性心肌缺血条件下对诱发的室性心律失常的影响;并探讨其抗心律失常作用的可能机制。方法:30只新西兰长耳大白兔被随机分为:正常组、丹参酮组和缺血组,每组10只。利用左室楔形心肌组织块的冠脉灌流建立兔急性缺血模型,正常组全程灌注正常台式液,缺血组在灌注90min正常台式液后停灌30min造成缺血,丹参酮组在灌注1h正常台式液后,换成含浓度3×10-6g/mL丹参酮ⅡA的台式液预处理30min。同步记录心肌组织块的跨壁心电图、内外膜的跨膜动作电位,在缺血30min时观察3组的QT间期、TDR(跨壁复极离散度)、APD90(内外膜的跨膜动作电位复极90%时程),程序性刺激并记录在缺血30min时心律失常的发生情况。结果:缺血状态下,正常组与缺血组相比较:心外膜APD90从216±20ms缩短至131±20ms (P <0.01);TDR由42±4ms增加至72±9ms(P <0.01)。缺血组和丹参酮组相比:心外膜APD90由131±20ms延长至155±23ms(P <0.05);TDR由72±9ms减少至57±5ms (P <0.05)。正常组没有诱发出心律失常,丹参酮组与缺血组诱发心律失常的发生率分别为20%(2/10)和90%(9/10)。结论:丹参酮ⅡA能改善在心肌缺血时的异常心电生理指标,显著减小TDR,并降低缺血时室性心律失常的诱发率。
【Abstract】 Objective: To study the effect of tanshinoneⅡA on ventricular arrhythmiasunder the condition of ischemic rabbit hearts, and to investigate its possible mechanismof anti-arrhythmia.Mehtods: Thirty New-Zealand long-eared white rabbits were fell intonormal group, tanshinoneⅡA group and ischemic group randomly, every group10rabbits.To establish ischemic modle by Rabbit left ventricular myocardial wedge ofcoronary perfusion. In normal group, perfused with Tyrode’s solution thoughoutly; inischemic group, Tyrode’s solution was perfused in initial90min and then stoppedirrigation in next30min; in tanshinoneⅡA group, Tyrode’s solution was perfused ininitial60min and3×10-6g/mL tanshinoneⅡA was instead to continued perfusing innext30min. The transmural ECG and action potentials from both endocardium andepicardium were simultaneously recorded. QT interval, TDR (the transmuraldispersion of repolarization), APD90, The incidence of ventricular arrhythmia by stringstimulation was observed after ischemia for thirty min.Results: Under the condition of ischemia, APD90of epicardium abbreviated216±20ms to131±20ms (P <0.01); TDR increased from42±4ms to72±9ms(P <0.01).Ischemic group compared with tanshinoneⅡA group, APD90ofepicardium increased from131±20ms to155±23ms(P <0.05); TDR decreasedfrom72±9ms to57±5ms (P <0.05). The incidence of ventricular arrhythmia innormal,tanshinoneⅡA and ischemia groups was0/10,2/10and9/10respectively. Conclusions: TanshinoneⅡA can improve the abnormal electrophysiologicalparameters, prevents ventricular arrhythmia and reduces TDR significantly inischemic rabbit hearts.
