细胞凋亡相关基因Bcl-2、Bax和核转录因子NF-KB在腮腺腺样囊性癌中的表达及临床意义

【摘要】 目的:观察Bcl-2、Bax和NF-KB在腮腺腺样囊性癌中的表达及其相关性,探讨三者与腮腺腺样囊性癌发生、发展及预后的生物学意义。方法:应用免疫组织化学SP法检测49例腮腺腺样囊性癌蜡块包埋组织中Bcl-2、Bax及NF-KB基因蛋白的表达情况,以20例正常腮腺组织作为对照。统计学方法采用χ2检验,统计学差异的显著性定义为p<0.05。结果:1、Bcl-2在腮腺腺样囊性癌组的阳性总表达率67.35%(33/49),明显高于正常腮腺组的表达率35.00%(7/20)(p<0.05);Bcl-2在腮腺腺样囊性癌TNM临床分期、淋巴结转移的表达统计学有差异(p<0.05);在腮腺腺样囊性癌不同病理类型的表达无差异(p>0.05)。2、Bax在腮腺腺样囊性癌组的阳性总表达率32.65%(16/49),高于正常腮腺组的表达率20.00%(4/20),但统计学比较无差异(p>0.05);Bax在腮腺腺样囊性癌病理类型、TNM临床分期、淋巴结转移的表达无差异(p>0.05)。3、NF-KB在腮腺腺样囊性癌组的阳性总表达率69.39%(34/49),明显高于正常腮腺组35.0%(7/20)的表达率,统计学分析有差异(p<0.05);NF-KB在腮腺腺样囊性癌TNM临床分期、淋巴结转移的表达统计学比较有差异(p<0.05);在腮腺腺样囊性癌病理类型的表达无差异(p>0.05)。4、Bcl-2、Bax两者在腮腺腺样囊性癌的表达过程中呈现负相关趋势(Kappa=-0.057),但统计学分析无差别(p>0.05)。5、Bcl-2和NF-KB两者存在相关性(p<0.01),且存在正相关关系(kappa=0.387)。结论:1、Bcl-2、NF-KB与腮腺腺样囊性癌的发生、发展、预后有着密切的关系,同时可以作为判断腮腺腺样囊性癌发生、发展、预后的重要指标。2、NF-KB通过调控并增加下游抗凋亡基因Bcl-2的表达在腮腺腺样囊性癌的发生、发展过程中发挥作用。3、Bax虽阳性表达增强,但与腮腺腺样囊性癌发生、发展及预后无关。
【Abstract】 Objective:To observe the expression and correlation of Bcl-2, Bax and NF-KB in the parotid ACC, explore the occurrence,development and prognosis of biological significance with ACC of the parotid gland.Methods:Detected the expression of Bcl-2, Bax and NF-KB gene protein in49cases of ACC wax block embedded tissue in Immunohistochemistry SP method,20cases of normal parotid tissue(NP) as control. Using x2test, the statistically significant difference is defined as p<0.05.Results:l.The total expression rate of Bcl-2in ACC group is67.35%(33/49), significantly higher than NP group (35.00%7/20)(p<0.05);The expression of Bcl-2in different clinical TNM stages and lymph node metastasis of ACC are difference (p<0.05);the expression of different pathological typing is no differences (p>0.05).2.The group’s total expression rate of Bax in ACC is32.65%(16/49), higher than NP group (20.00%4/20),but there was no difference (p>0.05);The expression of different pathological type, different clinical TNM stage, lymph node metastasis was no difference in ACC (p>0.05).3.The total expression rate of NF-KB in ACC group is69.39%(34/49), which is significantly higher than the rate of NP group (35.0%7/20)(p<0.05); The expression of Bcl-2in different clinical TNM stages and lymph node metastasis of ACC are difference (p<0.05);the expression of different pathological typing is no differences (p>0.05).4.There is negative correlation trend between the Bcl-2and Bax (kappa=-0.057),but no difference in the statistics (p>0.05).5. There ia some positive correlation between Bcl-2and NF-KB (p<0.01kappa=0.387). Conclusion:1. Bcl-2and NF-KB have close relation with the occurrence, development arid prognosis.They can be used as the important indicators of the occurrence,development and prognosis of ACC of the parotid giand.2. NF-KB plays a important role by up-regulating the expression of Bcl-2in the occurrence and development process of adenoid cystic carcinoma of parotid gland.3.The expression of Bax is positive in ACC of the parotid giand, But there was no difference in the statistics. There are no relationship between pathological type, clinical stage and lymph node metastasis.
