吉西他滨经动脉固定速率泵入与30min滴注治疗晚期胰腺癌比较
Effect of the Treatment of Advanced Pancreatic Cancer:a Comparison Between Gemcitabine Pumped into Artery at Fixed-speed and30Minute Standard Infusion Regimen

【摘要】 目的通过比较吉西他滨经动脉固定速率泵入方案和30min标准滴注方案治疗晚期胰腺癌的的疗效,评估吉西他滨经动脉固定速率泵入治疗胰腺癌的安全性、有效性,探讨吉西他滨经动脉泵入治疗胰腺癌的最佳固定速率。方法60例胰腺癌患者,随机分为标准组(1000mg/m2吉西他滨静脉滴注30min)、A组(1000mg/m2吉西他滨经动脉途径以5mg/m2.min)、B组(1000mg/m2吉西他滨经动脉途径以10mg/m2.min)、C组(1000mg/m2吉西他滨经动脉途径以15mg/m2.min),每组各15例。每个周期结束后评价患者血液学毒性、目标病灶体积变化、临床受益反应、生存时间。结果吉西他滨经动脉固定速率泵入方案与30min标准滴注方案治疗比较血液学毒性差异无统计学意义。吉西他滨经动脉固定速率泵入方案与30min标准滴注方案治疗比较生存期明显延长,差异有统计学意义。1000mg/m2吉西他滨经动脉途径以10mg/m2min固定速率泵入的CR+PR与30min标准滴注方案治疗比较明显提高,差异有统计学意义。结论1000mg/m2吉西他滨经动脉途径以10mg/m2min治疗晚期胰腺癌是安全、有效的,可以提高患者的生活质量,延长生存期。
【Abstract】 Objective Through comparing effect of the treatment of advanced pancreatic cancer:between Gemcitabine pumped into artery at fixed-speed and30min standard infusion regimen, the safety and effectiveness of the treatment of advanced pancreatic cancer by gemcitabine pumped into arteryl at fixed-peed were evaluated. The best fixed-speed of Gemcitabine pumped into artery were also discussed.Methods60cases of pancreatic cancer patients randomly were divided into standard group (the total does of Gemcitabine infused was1000mg/m2in30minute), A group (1000mg/m2gemcitabine were pumped into artery at5mg/m2.min), B group (1000mg/m2gemcitabine were pumped into artery at10mg/m2.min), C group (1000mg/m2gemcitabine were pumped into artery at15mg/m2.min), the cases of each group was15. Toxicity, target lesion volume change, clinical benefit response, survival time of patients were evaluated in the end of each cycle.Results There was no significant difference in the toxicity between gemcitabine pumped into artery at fixed-speed and30min standard infusion regimen in the treatment of advanced pancreatic cancer. The treatment of Gemcitabine pumped into artery at fixed-speed was better than the standard regimen in survival time(p<0.05). The rate of (CR+PR) in the treatment of Gemcitabine pumped into artery at fixed-speed was higher than that of the30min standard infusion and the difference was statistically siginificant.tarerial fixed speed pump into the scheme with 30min standard infusion regimen in the treatment of survival period was prolonged, the difference was statistically significant.Conclusion1000mg/m2gemcitabine pumped into artery at10mg/m2.min was safe and effective in the treatment of the anvanced pancreatic cancer It would improve the quality of life of patients, prolong the survival time.
