脂联素及受体在胃癌患者体内的表达水平和临床意义的研究
The Expression Level and Clinical Significance OD Adiponectin and Adiponectin Receptor in the Patients with Gastric Cancer

【摘要】 研究目的:脂联素(adiponectin)是主要由白色脂肪组织分泌的一种活性多肽,其在血浆中的含量相当丰富。近年来的研究表明,脂联素与肿瘤特别是与肥胖相关的生殖系统和消化系统的肿瘤密切相关。本研究通过检测胃癌患者及健康者血清中脂联素的浓度及脂肪组织中脂联素、脂联素受体1(adiponectin receptor1, AdipoR1)和脂联素受体2(adiponectin receptor2, AdipoR2)的表达,探讨脂联素及其受体与胃癌之间的关系,从而为胃癌的早期发现和治疗提出依据。研究方法:随机选取泰山医学院附属医院2009年2月至2011年2月住院的62例胃癌患者和31例健康人,收集新鲜血清应用酶联免疫吸附试验(enzyme-linked immunosorbentassay, ELISA)测定脂联素的浓度;收集腹内脂肪组织,运用聚合酶链式扩增(ReverseTranscription—Polymerase Chain Reaction,RT-PCR)以及蛋白质印迹免疫方法(Westernblot)检测脂联素、脂联素受体l和脂联素受体2的表达,分析其表达之间是否存在差异,以及不同肿瘤组织病理分期等因素与脂联素及其受体间的关系。结果:胃癌患者的血清脂联素水平(9.7±5.2ng/mL)明显低于健康对照组(13.9±8.5ng/mL)(P<0.05),同时脂肪组织中脂联素的表达水平也降低(P<0.05),且都与胃癌的病理分期呈负相关。在胃癌患者脂肪组织中均有AdipoR1和AdipoR2的表达,其中AdipoR1的表达水平较正常人群升高(P<0.05),但AdipoR2的表达水平不变。回归分析显示,低脂联素血症可能是胃癌发生的一个新的危险因素。结论:脂联素的表达水平降低与胃癌的发生发展有关,并且与胃癌的病理分期呈负相关;胃癌患者脂肪组织中均有AdipoR1和AdipoR2的表达,但只有AdipoR1的表达水平升高。意义:本实验提示我们脂联素及其受体在胃癌发展过程中起重要作用,脂联素表达下降是胃癌发生的一个独立危险因素。脂联素及其受体的检测可以为胃癌的预防、诊断及治疗提供新的方法和思路。
【Abstract】 Backgrouds Adiponectin, derived mainly from adipocyte, is present abundantly incirculating blood. It has been proved that adiponectin was closely associated with cancerespecially the obesity-related reproductive and digestive systems cancer. In this study, wedetermined the plasma levels of adiponectin in gastric cancer and healthy controlsrespectively, detected the expression of adiponectin receptor1and receptor2in theadipose tissue, investigated the relationship between gastric cancer and adiponectin andadiponectin receptor, thus to find out some evidences for early diagnosis and treatment ofgastric cancer.Methods First we selected randomly62patients who were diagnosed as gastric cancer atthe Affiliated Hospital of Taishan Medical College from February2009to February2011and31healthy controls. The concentration of adiponectin in two groups was detectedusing a specific ELISA assay, the expression of adiponectin receptor1and receptor2wasdetected using Polymerase Chain Reaction and Western blot, we also analyzed whetherthere have some difference in the expression between these two groups.Results The mean adiponectin levels in gastric cancer patients and healthy controls were9.7±5.2ng/mL and13.9±8.5ng/mL respectively, the plasma adiponectin concentration ofgastric cancer patients was significantly lower than the healthy controls, we found that thetissue level of adiponectin was much lower in cancer patients than in the healthy controlstoo. Meanwhile, the more serious of the patients, the lower of the adiponectin level in thetissue, it has a negative relationship with the Dukes’ classifations.We also found that there were both the expression of adiponectin receptor1and receptor2in neighboring noncancer tissues samples and gastric cancer tissues samples. Theexpression of receptor1was higher in gastric cancer patients than healthy controls, but theexpression of receptor2was unchanged. The regression analysis showed thathypoadiponectinemia may be a new risk factor for gastric cancer. Conclusion Our results showed that lower plasma levels of adiponectin were associatedclosely with gastric cancer. Meanwhile, the levels of adiponectin were negatively related tothe Dukes’ classifations of gastric cancer. There were both the expression of adiponectinreceptor1and receptor2in adipose tissues samples, but only the expression of receptor1was increased.Significance Our study investigated that both the adiponectin and adiponetin receptorplayed a key role in the progress of gastric cancer, the hypoadiponectinemia may be a newrisk factor for gastric cancer. Detection the levels of adiponectin and adiponectin receptorwill give us new methods and ideas for gastric cancer prevention, diagnosis and treatment.
