人膝骨关节炎骨赘OPN的表达及其意义

【摘要】 目的通过检测人膝骨关节炎骨赘中OPN的表达,探讨OPN在OA骨赘形成过程中的意义。方法选取接受膝关节镜手术和膝关节置换手术的原发性膝关节骨关节炎患者骨赘标本50份,取正常膝关节软骨(股骨髁关节面)10份作为正常软骨对照。采用综合评分法对OA患者病情进行严重程度分级,分为轻、中、重度三组;根据K-L分期将OA组患者分为两组。对标本进行免疫组织化学染色,用SPSS17.0统计软件包分析各组间OPN表达的差异及OA患者OPN表达与综合评分、K-L分期的相关性。结果人膝骨关节炎骨赘中OPN表达明显高于正常软骨组,统计有显著性差异(p<0.01);OPN在轻、中、重骨关节炎中的表达有显著差性异性(p<0.05);OPN在K-L分期组间表达有显著性差异(P<0.05)。骨赘中OPN的表达与骨关节炎患者的综合评分、K-L分期呈正相关。结论1.OPN在OA患者膝关节骨赘中有表达。2.OPN在中重度骨赘中高表达,且其表达与K-L分期正相关,提示OPN在骨赘发生过程中起作用。3.OA患者膝关节骨赘OPN表达与疾病严重程度正相关,提示OPN在骨关节炎发病过程中有作用。
【Abstract】 Objective:This study is designed to measure the contents of OPN in the osteophytes in patients with osteoarthritis of the knee,in order to investigate the significance in the development of osteophytes by examing the expression of OPN.Methods:The study was performed on50specimens of osteophytes obtaned from osteoarthritic patients who underment total knee arthoplasty or laparoscopic surgery. Take samples of the articular cartilage (femoral condyle)10cases of normal cartilage as a control.Then all the samples were ascribed to three groups of mild、moderate and severe group on the basis of the integrated rates. According to K-L grading, the OA patients will be divided into two groups The expression of OPN was detected in these samples by the immunohistochemistry and expression result statistics were analysed by SPSS17.0.Results:The osteophytes concentration of OPN in OA group was markerly higher than that in control with a significant difference(p<0.01).The concentrations of OPN were significant difference in mild, moderate and severe three groups(p<0.05), and between the two guoups which divided by K-L grading (P<0.05).The level of OPN expression in osteophyte and the integrated rates of osteoarthritis、K-L grading was positively correlated.Conclusion:1.The expression of OPN was found in osteophytes of OA patients.2.OPN is highly expressed in the moderate and severe osteophytes group, and its expression is associated with the KL grading, suggesting that OPN plays a role in osteophyte formation.3.In OA group, the level of OPN expression and synthesis rates are positively correlated, suggesting that OPN in the pathogenesis of osteoarthritis in effect.
