【摘要】 1.目的：探讨腹腔镜下胆囊切除术(laparosopic cholecystectomy,LC)术中行内镜下括约肌切开术(Endoscopic sphincterotomy, EST)取石,与LC术前行EST取石治疗胆囊结石合并胆总管结石相比的优缺点。2.方法：回顾性分析邵逸夫医院2009-2011年LC术中行EST及LC术前行EST的病例各30例,对比两者术后患者临床症状、并发症、实验室指标。3.结果：两组患者均治愈出院,LC术中组腹痛、高淀粉酶血症例数少于术前组(P<0.05)；LC术中组住院天数少于术前组(P>0.05)；LC术中组发热、出血、胰腺炎、结石残留发生例数少于术前组(P>0.05)；术中组白细胞升高例数多于术前组(P>0.05)；术中组总胆红素、谷氨酰转肽酶下降例数少于术前组(P>0.05)。4.结论：LC术中EST可以有效减少患者术后并发症及术后临床症状,并且明显缩短患者住院时间更多还原

【Abstract】 1.Objective:To analyze and compare the merits and faults of Preoperative EST and intraoperative EST.2.Methods:Post-hoc analysis patients with gallstones, gaven Preoperative EST or intraoperative EST, both for 30cases, they are collected from Sir Run Run Shaw Hospital 2009-2011. To compare their clinical symptom.lab test result, and complications.3.Results:All the patients of two groups are cured to discharge from hospital. The cases of abdominal pain, hyperamylasemia in intraoperative group is less than the Preoperative group(P<0.05); The intraoperative group has a shorter hospital stay(P>0.05); the cases of fever, haemorrhage, pancreatitis, residual of stones are less than the Preoperative group (P>0.05); The cases of WBC increasing are higher than the Preoperative group (P>0.05); The cases of TBIL.GGT decreasing are less than the Preoperative group (P>0.05),4.Conclusion:the intraoperative group has a lower clinical symptom, complications, and a shorter hospital stay.更多还原
