Pim-2、NF-κB在肝细胞癌中的表达和意义及两者相关性研究

【摘要】 目的:研究Pim-2、NF-κB在肝细胞癌(HCC)中的表达情况,根据表达情况探讨Pim-2、NF-κB与肝细胞癌的发生、临床病理特征及术后无瘤生存情况之间的关系,并探讨Pim-2、NF-κB在HCC表达的相关性。方法:采用免疫组化(SABC法)分别检测60例HCC组织及相应的癌旁组织和12例正常肝组织中Pim-2、NF-κB的表达。比较不同组织的表达差异,结合临床病理资料及随访资料分析两者表达与肝癌患者临床病理特征及术后无瘤生存时间的关系。并对二者在HCC中表达的相关性进行研究。结果:1.12例正常肝组织中Pim-2、NF-κB表达均为阴性。60例HCC组织中34例Pim-2呈阳性表达,阳性率为56.7%,60例癌旁组织中18例Pim-2呈阳性表达,阳性率为30%,组间有显著差异(p<0.05)。60例HCC组织中46例NF-κB表呈阳性表达,阳性率为76.7%,60例癌旁组织中28例NF-κB呈阳性表达,阳性率为46.7%组间有显著差异(p<0.05)。HCC组织中Pim-2、NF-κB的表达均高于相应的癌旁组织和正常肝组织(P<0.05)。2.Pim-2、NF-κB在肝癌组织的表达与患者的年龄、性别无关(p>0.05),与肝癌的病理分级、侵袭转移、乙肝感染、AFP、肝硬化及child分级有关(p<0.05)。3.肝癌组织中Pim-2与NF-κB的表达呈正相关性(r=0.733,p=0.000)。4.癌旁组织中Pim-2、NF-κB的表达均与无瘤生存时间密切相关。Pim-2阳性表达组中位无瘤生存时间为14个月,阴性表达组为23个月,两者相比较有显著性差异(p<0.05)。NF-κB阳性表达组中位无瘤生存时间10个月,阴性组为25个月,两者相比较有显著性差异(p<0.05)。5.癌旁组织中NF-κB、Pim-2双阳性组的中位无瘤生存时间为8个月,NF-κB+、Pim-2-组为11个月;NF-κB-+、Pim-2+组为15个月,F-κB、Pim-2双阴性组为31个月,各组间有显著差异(p<0.05)。结论:1.Pim-2、NF-κB高表达于HCC组织中,表明Pim-2、NF-κB可能参与肝癌的发生发展。2. Pim-2、NF-κB在HCC的表达呈正相关,提示两者在肝癌的发生、发展中可能存在协同作用。3. Pim-2、NF-κB在癌组织及癌旁组织中的表达,与肝癌患者术后无瘤生存期有关,可作为肝癌预后的参考指标。

【Abstract】 objective:To investigate the expression of Pim-2 and NF-KB in HCC, and explore their relationship with tumorigenesis、clinicopathologic features and prognosis in HCC.Try to discover the correlation of Pim-2 and NF-KB in HCC.Methods:Using immunohistochemical method (SABC) to detect the expressions of Pim-2 and NF-κB in 60 HCC tissues,60 para-carcinoma tissues and 12 normal liver tissues.Compare the difference of their expressions among these tissues,and analyze their relationship with clinicopathologic features and disease free survival time in HCC.Results:1.There is no positive expression in normal liver tissue, the positive expression rate of Pim-2 in HCC was 56.7% and the positive expression rate of Pim-2 in the para-tumor tissues was 30%(p<0.05); NF-κB expression in HCC and para-tumor tissues was 76.7% and 46.7% respectively (p<0.05). Both the positive expression of Pim-2 and NF-κB in HCC is obviously higher than the positive expression of Pim-2 and NF-κB in paratumor tissues and normal liver tissue.2. the expression of both Pim-2 and NF-κB is not statistically related with gender, age(p>0.05),while closely related with Edmondson grade、metastasis、hepatitis B、AFP elevation、child grade and hepatocirrhosis of HCC (p<0.05). 3 The expression of Pim-2 and NF-κB is positively correlated in HCC.4. Pim-2 expression in paratumor tissue is closely related with disease-free survival time. The medians for disease-free survival time in the positive expression group is 14 months, while the negative expression group is 23 months. The medians for disease-free survival time in NF-κB positive group is 10 months, while in the negative group is 25months.Cumulative survival curves of the Pim-2+group is significantly lower than the Pim-2.-group (p<0.05), it is the same with NF-κB.The expression of pim-2.、NF-κB is associated with the disease-free survival time.5. The medians for disease-free survival time in the group which both Pim-2 and NF-κB are positive in paratumor tissue is 8 months,the Pim-2-、NF-κB+ group is 11 months,the Pim-2+、NF-κB- group is 15months and the Pim-2-、NF-κB- is 31 months,the statistical difference among the groups is obvious(p<0.05).Conclusion:1.Pim-2, NF-κB is highly expressed in HCC, indicating that Pim-2, NF-κB may be associated with the occurrence and progress of HCC.2.The expression of Pim-2、NF-κB is positively correlated each other in HCC, suggesting that they have synergistic effect in the occurrence and progress of HCC.3.The expression of.Pim-2、NF-κB in HCC and paratumor tissue is associated with the disease-free survival time, they can be predicators of HCC prognosis.
