【摘要】 目的系统性红斑狼疮(systemic lupus erythematosus,SLE)是一种自身免疫性疾病,以多种病理性自身抗体的产生、免疫复合物沉积及多系统损害为特征,其中以肾脏受累最为常见。研究发现SLE患者体内补体被强烈活化,,病变部位有补体沉积,表明补体在SLE时有病理损伤作用。补体H因子(Complete factor H, CFH)是一种重要的补体调节物质,它决定补体C3b的命运,控制C3转化酶的生成和稳定性。通过检测系统性红斑狼疮(SLE)患者血清补体因子H(CFH)的表达水平,探讨其水平变化与SLE患者其他实验室检测指标关系以及其在SLE发病中的作用。方法选取符合1982年美国风湿病学会制定的SLE诊断标准的37例SLE患者,另选30例健康人作对照,采取酶联免疫吸附法检测SLE组及对照组血清中的CFH的水平,同时检测临床相关指标，分析CFH在各组患者血中的变化及其与临床各指标的相关性。结果SLE患者血清CFH水平为(332.96±135.27)ng/ml,低于正常对照组水平(414.81±72.79)ng/ml, [P<0.05]。SLE血清CFH水平与补体C3呈明显正相关(r=0.518,P<0.001)，与ds-DNA,尿微量白蛋白,SLEDAI评分呈负相关(r=-0.425,P<0.01；r=-0.472,P<0.005；r=-0.366,P<0.05)。结论CFH的减少可能参与SLE的发病过程,特别与SLE造成肾脏损伤有关,是导致SLE患者低补体血症的因素之一,在一定程度上反映疾病活动。更多还原

【Abstract】 AIMTo investigate the relation between the change of serum Complete factor H (CFH) level and other laboratory indexes by detecting serum CFH level from patients with systemic lupus erythematosus(SLE) and explore the role of CFH in SLE.METHODSThe level of serum CFH from 37 SLE patients diagnosed according to 1982 ACR diagnostic criteria for SLE and 30 healthy persons were detected by enzyme-linked immunosorbent assay(ELISA)respectively.Meanwhile, other laboratory indexes were detected and the relation between the change of serum CFH level and other indexes were analyzed.RESULTSThe level of serum CFH in patients with SLE(332.96±135.27ng/ml) was obviously lower than that in healthy control(414.81±72.79ng/ml)(P<0.05).The levels of serum CFH in SLE patients showed positive correlation with C3 level(r=0.518, P<0.001) and showed negtive correlation with ds-DNA, urine albumin and SLEDAI activity integra (r=-0.425, P<0.05; r=-0.472, P＜0.05; r=-0.369, P<0.05)。ConclusionComplete factor H deficiency may be involved in the pathogenesis of SLE, espacially in the kidney impairment,and contributes to the hypocomple--mentemia.In addition,it can also reflect the degree of SLE activity to a certain extent.更多还原
