术后阿片类药物代谢和μ-阿片受体基因(OPRM1)A118G位点多态性之间关系的meta分析

【摘要】 目的:分析术后病人阿片类镇痛剂的用量及疼痛程度与μ-阿片受体基因(μ-opioid receptor gene,OPRM1) A118G单核苷酸多态性(single nucleotide polymorphism, SNP)之间的关系。方法:根据纳入和排除标准,通过检索Pubmed. Embase和ISI Web of Science数据库,检索截至2010年2月28日已发表的所有文献。利用meta-分析的方法研究术后病人阿片类镇痛剂的用量及疼痛程度和OPRM1 A118G SNP之间的关系。结果:共纳入8个研究(1552例病例)。带有A等位基因的病人术后需要阿片类镇痛剂的剂量比带有G等位基因的病人少(P<0.05,标准化均数差(standardized mean difference, SMD)值及其95%置信区间(confidence index, CI)不包含0)。而A118G多态性和病人的疼痛程度无关(P>0.05,加权均数差(weighted mean difference, WMD)值及其95%CI包含0)。结论:A118G SNP和术后病人的阿片类镇痛剂需求量相关,但和病人的疼痛程度无关。

【Abstract】 Objective:To investigate the potential association among opioid analgesics requirement and pain tolerance and A118G genetic polymorphism ofμ-opioid receptor gene (OPRM1) after surgery.Methods:According to study criteria, literature searching was conducted in Pubmed, Embase and ISI Web of Science to identify studies published before Feb.28,2010. We used meta-analysis to examine the association.Results:This meta-analysis included 8 studies with 1552 patients. Patients with the A allele required less opioids analgesics than with the G allele of the A118G single nucleotide polymorphism (SNP) (P<O.05, standardized mean difference (SMD) with 95% confidence index (CI)<0). There was no association between the SNP and pain tolerance (P>0.05, weighted mean difference (WMD) with 95% CI passed the zero line).Conclusion:Our meta-analysis of available studies indicated that the OPRM1 A118G polymorphism was associated with a significant variability in response to opioid analgesics, rather than pain tolerance, with patients receiving treatment for postoperative pain.
