白色念珠菌系统性感染机体免疫功能变化的实验研究

【摘要】 目的:探讨念珠菌系统性感染机体Th细胞免疫功能的特点,以及细胞因子与组织病理变化的关系。方法:通过尾静脉注射一定数量的念珠菌孢子,用平皿稀释法检测内脏组织真菌载量(CFU/g);HE染色和PAS染色进行内脏组织病理学检查;采用Con-A刺激脾细胞,酶联免疫吸附法(ELISA)检测诱生的细胞因子(IFN-γ和IL-4),以及用ELISA法检测肾组织IL-6水平。动态观察感染时间对各内脏(肝、肾、脾)组织病理变化的影响,念珠菌感染量与Th细胞的免疫反应类型;IL-6的表达水平与组织病理变化的关系。结果:接种致死量时,随着感染时间的延长肾组织真菌载量增加,念珠菌菌丝相明显,肾脏的病理损伤严重。接种非致死量时,在感染3天后肾组织内真菌基本被完全清除。而在不同感染量时,脾和肝组织病理变化不明显,随着时间的延长(3天后),念珠菌可完全被清除。无论接种致死量还是非致死量的白色念珠菌,诱生的IFN–γ明显降低,IL-4明显升高;而肾脏的IL-6水平,在接种非致死量时,随感染时间延长呈上升趋势;接种致死量时,感染初期IL-6水平呈上升趋势,感染后期呈下降趋势。结论:在系统性念珠菌感染中,各组织对病原体的易感性存在差异;无论致死量还是非致死量的白色念珠菌感染,机体呈现以Th2细胞为主的免疫反应;IL-6在白色念珠菌性肾炎发生发展过程中可能发挥双向作用。
【Abstract】 Objective To determine the type of Th cell immune responses of systemic fungal infection and the relationship between cytokine and organ histopathological changes.Methods A murine model of systematic Candidiasis was established by injection with Candida albicans (C. albicans)via the tail vein.. Organ fungal burden were determined with the plating dilution method. Sections of internal organs were made and stained by HE & PAS staining. Histopathological changes were inspected. Cytokines (IFN-γand IL-4) induced by Con-A in splenic lymphocyte and IL-6 from kidney tissues were detected by ELISA (Enzyme–linked immunosorbebt assay).Results Fungal colony counts in the kidney progressively increased during fatal infection and decreased during nonfatal infection. The hypha of C. albicans was obvious in the kidney. The histopathological damage was serious. In contrast, the colony counts in spleen or liver progressively declined in all mice and the histopathological changes were not obvious. Induced splenic IFN–γdecreased, while IL-4 increased sharply during fatal or nonfatal infection. In nonfatal infection group, IL-6 of kidney presented a tendency of increasing. While in fatal infection group it presented a tendency of increasing and decreasing respectively during earlier and later period.Conclusion There are susceptivity differences of different organs in systematic infection of C. albicans. It indicates a Th2 type in systemic candidiasis. IL-6 may play bidirectional roles during the period of candidiasis nephritis formation and development.
