【摘要】 目的:分析胃肠道癌术后患者外周血淋巴细胞P16、cyclin Dl表达的临床意义。方法:43例健康人设为对照组,胃肠道癌术后一年患者52例,根据影像学检查结果及体征按是否发生复发、转移分成稳定组(35例)和恶化组(17例)。用流式细胞仪检测其外周血淋巴细胞P16和cyclinDl的表达,用Kruskal-WallisH检验、mann-whitneyU检验分析健康对照组、恶化组及稳定组外周血淋巴细胞P16、cyclin Dl表达的差异性;并对恶化组外周血淋巴细胞P16与cyclinD1表达水平做Spearman秩相关;绘制ROC曲线,确定临界值,计算P16、cyclin Dl的特异度和灵敏度。结果:三组中外周血淋巴细胞P16之间和cyclinD1之间存在差异性(P < 0.001),其中恶化组P16表达和cyclinD1表达均高于稳定组及健康对照组(P < 0.001);稳定组cyclinD1表达与健康对照组无明显差异(P >0.05),稳定组P16表达高于健康对照组(P < 0.001),且恶化组外周血淋巴细胞P16及cyclinD1蛋白表达呈正相关,相关系数为0.55。绘制ROC曲线,确定复发临界值:P16为6.71 ;cyclinD1为6.22。灵敏度、特异度为:P16 82.4%、80.0%;cyclinD1 76.5%、85.7%;两指标联合检测并联试验灵敏度达82.4%,串联试验特异度达94.3%。结论:外周血淋巴细胞P16、cyclin D1高表达对胃肠道癌术后复发的诊断及判断预后有重要的临床价值。更多还原

【Abstract】 Objective: To analogy the clinical significance on the expression of P16 and cyclinD1 in blood lymphocyte in postoperative patient with gastrointestinal cancer.Methods: Flow cytometry was used to detect the expression of the P16 and cyclinD1 in blood lymphocyte in three groups:43 healthy peoples for control group,52 patients who were operated as gastrointestinal cancer one year ago,were divided into stable group and advanced group according to the clinical outcome. The expression in the three groups was analogy by Kruskal-Wallis H test and mann-whitney U test;Darw the Roc curve to definite the curital value ,and calculate the sensitivity and specificity of the P16 and cyclinD1.Results: The expression of the P16 and cyclinD1 in peripheral blood lymphocyte were significantly different in the three groups(P < 0.001),and the expression of the P16 and cyclinD1 in advanced group was significantly higher than those in control group and stable group(P < 0.001), the expression of the cyclinD1 in stable group had no significant difference with control group(P >0.05),but The expression of the P16 in stable group was higher than that in control group(P < 0.001).Draw the Roc curve to definite the critical value of recidivision:P16 was 6.71,cyclinD1 was 6.22;The sensitivity and specificity ;P16 was 82.4%、80.0%,respectively,cyclinD1 was 76.5%、85.7%,respectively.Furthermore,the sensitivity and specificity of the combinative detection of the two index was 82.4%、94.3%,respectively.Conclusion: It is important that The high expression of the P16 and cyclinD1 in blood lymphocyte to judge of the recidivation and prognosis of gastrointestinal cancer.更多还原
