春源金属硫蛋白（MT）驱铅片治疗铅中毒的临床研究

【摘要】 目的:观察金属硫蛋白驱铅片对铅中毒儿童血铅水平,血锌、铁、钙、镁、铜含量,血红蛋白的影响,以及对临床症状的改善情况,探讨MT对儿童铅中毒的治疗效果及可能的作用机制,对治疗、预防儿童铅中毒提供依据。方法:将240例铅中毒儿童随机分为2组,实验组口服金属硫蛋白驱铅片,对照组口服安慰剂普通蛋白片。口服金属硫蛋白驱铅片或安慰剂1个月和2个月后,检测两组儿童的全血铅水平,全血锌、铁、镁、钙、铜及血红蛋白含量,对治疗前后临床症状进行评估,并分析它们的变化。结果:实验组儿童服用金属硫蛋白驱铅片2个月后血铅水平(μg.L-1,65±16)明显低于对照组（112±24）（P＜0.01）;实验组血锌、铁（P＜0.01）、镁含量明显升高（P＜0.05）,而血钙、血铜水平无明显改变（P＞0.05）;治疗后,实验组血红蛋白明显升高（p＜0.01）;实验组临床症状的改善情况明显优于对照组;实验组118例患儿红细胞与白细胞计数、心电图,及肝肾功能,在治疗前后均无明显改变（P＞0.05）。结论:金属硫蛋白驱铅片改善临床症状与提高血红蛋白的作用明显优于对照组,且无不良反应金属硫蛋白驱铅片可降低铅中毒儿童血铅水平,并使体内元素保持平衡,对铅中毒儿童是安全、有效的。
【Abstract】 Objective To observe the effect of metallothionein（MT） lead-expelling tablets on blood lead levels,contents of trace elements Zn、Fe、Ca、Mg、Cu and Hb in lead poisoning children and the improvement of clinical syndrome.To study the mechanism and therapeutic effect of metallothionein on lean poisoning children.To provide evidence for the prevention of lead poisoning.Methods Two hundred and forty cases of lead poisoning children were divided into two groups randomly.MT lead-expelling tablets were given to the experiment group,placebo was given to control group.After oral administration of MT lead-expelling tablets or placebo for 1 and 2 months, blood lead levels,contents of elements Zn、Fe、Ca、Mg、Cu and Hb were determined.Evaluate the clinical syndromes before and after therapy.Results After 2 months treatment,the blood lead level(μg·L-1,65±16) in the experiment group were obviously lower than that（112±24）of in control group（P＜0.01）,and the contents of blood zinc,iron（P＜0.01）and magnesium（P＜0.05） were increased,while contents of blood calcium and copper were not changed（P＞0.05）.The level of Hb in experiment group increased much after therapy（P＜0.01）.The improvement of clinical syndromes in the experiment group is priory to control group.The red blood and white blood count,the electrocardiograph,the function of liver and kidney all did not change much in experiment group of the a hundred and eighteen children（P＞0.05）.Conclusion The improvement of clinical syndromes and HB of MT lead-expelling tablets in experimental group is priory to that of in control group without side effect.MT lead-expelling tablets can decrease the blood lead levels,maintain the balance of elements in lead poisoning children and increase Hb.It is an effective and safe medieine in the treatment of children with lead poisoning.
