【摘要】 目的:观察超声乳化白内障吸除联合后房型折叠式人工晶状体植入术治疗合并白内障的急、慢性闭角型青光眼的疗效和安全性。方法:选择2007年1月至2009年1月收住本院的原发性闭角型青光眼34例(共41眼),其中急性闭角型青光眼(以下简称急闭)临床前期15眼,发作期10眼,慢性闭角型青光眼(以下简称慢闭)16眼。术前视力范围:光感～0.6,均有不同程度的晶状体混浊。入院后行超声乳化白内障吸除联合后房型折叠式人工晶状体植入术,随访时间为3个月～12个月,平均随访时间为(6.9±2.0)个月,随访内容包括视力、眼压、房角镜检查、前房深度等。结果:①41眼原发性闭角型青光眼术后37眼(90.2%)最佳矫正视力提高,30眼(73.1%)矫正视力>0.5;②急闭临床前期组术后平均眼压下降(t=7.094,P<0.05)。急闭发作期组术后平均眼压比术前明显下降(t=3.495,P<0.01)。慢闭组术后平均眼压比术前明显下降(t=3.667,P<0.01),差异均有统计学意义;③急、慢闭组术后房角关闭所在象限有不同程度开放;④中央前房深度急、慢闭组术后较术前明显增加(P<0.05);周边前房深度两组术后均较手术前增加。结论:采用超声乳化白内障吸除联合人工晶体植入术治疗合并白内障的闭角型青光眼是安全、有效的。更多还原

【Abstract】 Objective : To evaluate the efficacy and safety of phacoemulsification with foldable posterior chamber intraocular lens (PC-IOL) implantation in the management of acute or chronic primary angle closure glaucoma(APACG or CPACG)with cataract.Methods:Collect 34 cases(41eyes) of PACG with cataract during January 2007 to January 2009,including 15 eyes of preclinical APACG,10 eyes of paroxysmal APACG, 16 eyes of CPACG.Visual acuity distributed from light perception to 0.6.Phacoemulsification and intraocular lens implantation were performed and patients were followed up for (6.9±2.0)months about visual acuity ,gonioscopy, anterior chamber depth. Results:In 41 eyes of PACG,visual acuity was corrected in 37 eyes(90.2%),including 30 eyes (73.1 %)with corrected visual acuity>0.5;In preclinical APACG group,postoperative IOP decreased (t=7.094,P<0.05);in paroxysmal APACG group, postoperative IOP decreased (t=3.495,P<0.01);in CPACG groups,postoperative IOP decreased (t=3.667,P<0.01);In APACG and CPACG groups,angle of anterior chamber was widen. Preoperative central and peripheral ACD of APACG and CPACG were much less than those of postoperative (P<0.05).In 41 eyes of PACG,visual acuity was corrected in 37 eyes(90.2%)including 30 eyes (73.1%)with corrected visual acuity>0.5. Condusion:It’s safe and effective treat angle-closure glaucoma with cataract by phacoem -ulsification plus foldable IOL implantation.更多还原
