针灸治疗功能性消化不良现代文献数据挖掘

【摘要】 目的:检索国内外1970-2007.3针灸治疗功能性消化不良的现代文献,分析针灸处方,探求针灸治疗该病选用经络、腧穴的规律;以该病为载体,挖掘针灸临床取穴的一般规律。方法:计算机检索OVID MEDLINE(1970-2007.3)、PubMed(1980-2007.3)、EMbase(1984-2007.3)、EBM(包括Cochrane DSR、ACP Journal club、DARE、CCTR)、CNKI(1979-2007.3)、CBMdisc(1970-2007.3)、VIP(1989-2007.3)、万方数据库(包括学位论文与会议文献),手工检索《中国针灸》等九种国内期刊,凡是以人体十四经络腧穴为作用点的治疗方法治疗功能性消化不良的临床报道文献,包括临床试验、临床经验、病案报道、专家意见等均纳入。由两位研究者独立地对文献进行质量评价和资料提取,并进行交叉核对。对所搜集、入选的合格文献进行数据挖掘、分析腧穴运用规律、经脉选取关联和特定穴选取关联规律。结果:①腧穴选用结果:共使用腧穴46个,运用总频次320次;频次分析结果显示使用5次以上的腧穴有足三里、中脘、内关、胃俞、天枢等18个;腧穴分部关联分析结果显示处方中运用最多为下肢腧穴,其次依次为胸腹部腧穴、背部腧穴、足部腧穴、上肢部腧穴及手部腧穴;腧穴配伍分析结果显示处方中支持度最高为中脘(?)足三里,其次为足三里(?)内关、中脘(?)内关等。②经脉选用结果:针灸治疗FD选用经脉涉及十四经中的12条经脉,未选用手少阳三焦经及手太阳小肠经腧穴。选取的腧穴主要集中在胃经、任脉、膀胱经和心包经上。③特定穴选用结果:46个经穴中特定穴37个,占80.43%;按特定穴类别进行分析,结果显示运用最多为五输穴,其次为下合穴、募穴、交会穴、八会穴、背俞穴、络穴、八脉交会穴、原穴、郄穴;按具体特定穴进行分析,结果显示使用频次超过10次的特定穴为:胃合穴(下合穴)、胃募穴(腑会穴)、心包络穴(八脉交会穴通阴维)、胃背俞穴、大肠募穴、肝原穴、肝输穴、脾背俞穴;特定穴配伍分析显示支持度最高为背俞穴(?)五输穴,其次为五输穴(?)原穴、五输穴(?)络穴、背俞穴(?)络穴等。结论:针灸治疗功能性消化不良,取穴遵循“循经取穴”的基本原则,而穴位配伍则体现出经气会聚的特定穴为关键,这一结果将为针灸临床治疗FD的选穴提供文献依据。更多还原

【Abstract】 Objective:We searched for clinical trails which used acupuncture and moxibustion to treat functional dyspepsia before March,2007,the language was limited to English and Chinese and no matter the state.Analysis prescription,then summarize the regularity of Applying of fourteen meridians and points.Just use FD as the carrying agent to dig the general regularity in clinical trails.Methods:Retrieve OVID MEDLINE(1970 to March,2007),PUBMED(1980 to March,2007), EMBASE(1984 to March,2007),CMBdisc(1970 to March,2007),CNKI(1979 to March,2007),VIP(1989 to March,2007),wanfang database etc.Clinical trials which were published in acupuncture journals and other important journals were searched also.Intervention including acupuncture,moxibustion,electric acupuncture,dermal needle,Transcetaneous Electrical Stimulation,Laser acupuncture and so on.Eligible studies were included.Two reviewers assessed the methodological quality and exacted the data independently.Extracting the data of fourteen meridians in these studies.Counted and analyzed the regularity of acupoints,meridians and special acupoints in Data excavation process.Result:①The analysis of acupoints.46 acupoints used 320 times,the result of frequency suggest that zusanli(ST36)、zhongwan(CV12)、neiguan(PC6)、weishu(BL21)、tianshu(ST42) are mostly used;Association analysis of subdivision suggest that the most acupoints on the lower limbs,then breast,abdomen,back,foot,extremitas supedor and hand;Compatibility analysis of acupoints suggest that the topmost sustain is zhongwan(?) zusanli.②The analysis of channel:The Sunjiao Channel of Hand-Shaoyang and The Small Intestine Channel of hand-Taiyang are not included.Acupoints concentrate on The Stomach Channel of Foot-Yangming,Renmai,The urinary bladder Channel of Foot-Taiyang and The Pericardium Channel Of Hand-Jueyin.③The analysis of special points,37 were specific acpoints,the analysis of category of special acupoints suggest that Five Shu Points are mostly used,then lower He Point,Eight Meeting Point,Intersection Points etc;the lower He point of stomach is mostly used;Association Rule Mining of specific acupoints suggest that the topmost sustain is Back Shu Point(?) Five Shu Points,then Five Shu Points(?) Source Point,Five Shu Points(?) Network Point.Conclusions:The good therapeutic efficacy of treating FD by acupuncture and moxibustion is just established on the" selection of points on the affected channel" and "the convergence of meridian-Qi ",the conclusion will prodive literature evidence in electing meridian and point in clinical practice.更多还原
