角质细胞生长因子及E-钙粘蛋白在胰腺癌中的表达及其意义

【摘要】 目的:探讨胰腺癌组织中角质细胞生长因子(KGF)及E-钙粘蛋白(E-cadherin)的表达情况,研究二种指标与胰腺癌的临床分期、组织分化程度、淋巴结转移及性别、年龄等的关系。方法:应用免疫组化SP法检测45例胰腺癌组织、22例癌旁正常胰腺组织中KGF、E-cadherin的表达,并与临床病理资料作对照分析。结果:(1)KGF在胰腺癌组织中的阳性表达率为77.8 %(35/45),在癌旁正常胰腺组织中阳性表达率为9.09%(2/22),两组相比差异具有显著性(P<0.01)。(2) E-cadherin在胰腺癌组织和癌旁正常胰腺组织中阳性表达率分别为35.6%(16/45)、95.5%(21/22) ,两组相比差异具有显著性(P<0.01)。(3) KGF的表达在不同的性别、年龄、肿瘤部位、肿瘤大小、淋巴结转移之间无显著性差异,而与癌组织分化程度及临床分期有关。(4) E-cadherin的表达在不同的性别、年龄、肿瘤部位、肿瘤大小之间无显著性差异,而与癌组织分化程度、淋巴结转移及临床分期有关。(5) KGF和E-cadherin之间存在负相关关系(χ2 =8.730, P < 0. 05),二者关联性密切程度采用Pearson列联系数值C的大小衡量,计算得C =0.445,从而证明两者关系较密切。结论:(1) KGF在胰腺癌组织中高表达,并且与癌组织分化程度及临床分期有关,随胰腺癌组织分化程度进展及临床分期增加而升高。KGF的测定有助于判断胰腺癌的恶性生物学行为。(2)胰腺癌组织中E-cadherin的表达明显减弱,且与胰腺癌组织学分级、淋巴结转移、临床分期关系密切,表明E-cadherin与胰腺癌的转移、浸润、生长和粘附有关,可能是一个重要的肿瘤标记物。(3)KGF与E-cadherin的表达水平有望作为胰腺癌的增殖、分化、侵袭以及转移的重要生物学指标。

【Abstract】 Objectives To study the expression of KGF and E-cadherin in pancreatic adenocarcinoma and their clinical significance.Methods The expression of KGF and E-cadherin in 45 specimens of paraffin-embeded pancreatic adenocarcinoma and 22 normal pancreas around tumors was detected by using immunohistochemistry staining(SP) and compared with clinicopathological data.Results (1) The positive rate of KGF expression in pancreatic adenocarcinoma was77. 8 % (35/ 45), and that in normal pancreatic tissue was 9.09%(2/22). There was statistical difference between them(P<0.01). (2) The positive rate of E-cadherin expression in pancreatic adenocarcinoma and in normal pancreatic tissue was 35.6%(16/45)、95.5%(21/22),respectively. There was statistical difference between them(P<0.01). (3) There was no significant difference among the expression of KGF in different sex、ages、tumor location、tumor diameter of the cancer and the metastasis of lymph nodes, however there was a relationship with the differentiation of carcinoma and the clinical stages. (4) There was also no significant difference among the expression of E-cadherin in different sex、ages、tumor location and tumor diameter,however there was a relationship with the differentiation of carcinoma , the metastasis of lymph nodes and the clinical stages. (5) The expression between KGF and E-cadherin had a significantly negative relation in pancreatic adenocarcinoma (χ2=8.730, P<0.01). Relevance of them was measured by Pearson in numerical, C =0.445.Conclusions (1) KGF was highly expressed in pancreatic cancer and the expression of KGF was associated with the differentiation of carcinoma and the clinical stages, the expression of KGF was increased with the degradeing differentiated degree and the heightening the clinical stages,which indicated that it might be a useful tumor marker to assess the tumor’s malignant behavior.(2) The expression level of E-cadherin in pancreatic carcinoma was signifcantly decreased compared with that in nearby tumor free tissues, and it was significantly correlated with the pathological grading, lymph node metastasis and the clinical stage, which indicated that E-cadherin was correlated with the tumor metastasis, proliferation, cancer progression and tumor cell adhesion of pancreatic carcinoma and it might be a useful tumor marker of pancreatic carcinoma.(3) The level of KGF and E-cadherin may be a sensitive predictor of proliferation ,differentiation , invasion and metastasis of pancreatic adenocarcinoma.
