大竹中医院PACS系统研究

【摘要】 图象存档及通信系统(PACS)实现了临床医学图象的有效管理和共享,目前已在国内大中型医院得到了比较广泛的应用。随着信息传输技术的不断发展,医院PACS系统的建立具备了良好的物质基础,相对高速和廉价的宽带网技术使医学图象海量数据的远距离传输成为可能。而基层医院的PACS规划建设也已经纳入议事日程。DICOM标准是医疗设备的国际标准通讯协议,医学图像的数据通讯都是遵循DICOM标准的。目前国外的医疗设备厂商一般都以许可证方式提供符合DICOM标准的医疗设备,以解决不同厂商的各种医疗设备的互联问题。随着越来越多的医院对PACS系统的认识和应用,各大中型医院在购置新的医疗成像设备时,把能否提供符合DICOM标准的网关看作是一个重要的选型指标。HL7是基于国际标准化组织(ISO)所公布的网络开放系统互连模型(OS1)第7层(应用层)的医学信息交换协议。在医疗信息交换标准HL7(Health Level Seven)被广泛应用后,医疗部门间的互联互通和信息共享进入了一个新的时期。我国目前也在积极地进行着医疗信息系统的HL7标准化工作。论文首先综述PACS、DICOM、HL7基础理论知识及其国内外发展现状,概述了HIS和影像工作站的建设需求。接着根据大竹中医院实际情况,详细介绍了HIS升级、RIS建设及工作站的安全维护。最后阐述了中医院的非DICOM医疗设备的PACS接入,影像质量控制系统的设计,采用基于C/S(Client-Server)和B/S(Browser-Sever)相结合的分布式结构和HL7/DICOM网关模式集成HIS/PACS,在此基础上架构中医院PACS系统。然后介绍了HL7/DICOM网关的信息交换功能,分析了各种存储模式并提出中医院三级存储方式。该图像存储与通信系统PACS,包括其功能模块和使用方法,在县区级医院的信息化及其数字化建设过程中,具有很好的推广应用和参考。
【Abstract】 PACS (Picture Archiving and Communication Systems) can realize efficiency management and share of medical image。It has already got more extensive application in Large and medium-scale hospitals of China at present。Development of information transmission technology lays important foundation of PACS technology. High speed and cheap wide band web make it possible to transmit a mass of medical image in long distance. Based on the clinical demand, the PACS system has been incorporated into the planning and construction of the agenda in Primary Hospitals。DICOM is an international communication standard which conformed by manufacturers of medical apparatus。The communications of medical image conformances the standard。To solve the problem caused by the apparatus of different manufacturers when they are communicating each other, the foreign manufacturers provide their medical apparatus which conformance DICOM standard with license。With more and more user of PACS, the DICOM Gate is look as a important properties of PACS when user select it。Health Level Seven was a medical information exchange protocol, which was based on the 7th level of OSI. by ISO。Interconnection between the medical department’s exchanges with the information shares enters one new time after the extensive application of the healthcare communication standard-HL7 (Health Level Seven). Nowadays, our country is actively putting up the HL7-standardization of healthcare information system。The basic theoretical knowledge of PACS, DICOM and HL7 and its development in the world are introduced at first and then explained the construction demand of HIS and image workstation. And then, based on the actual situation of Dazhu Chinese medicine hospital, it developed a detailed account of the Design HIS for upgrade、RIS development scheme and the maintenance of workstation security。Finally expound that the non-DICOM medical equipment of it switch in PACS, image quality control system design, it based on the Client-Server and Browser-Sever combination of distributed architecture and HL7/DICOM Gateway Model Integration HIS/PACS, on the basis of this framework the PACS of Dazhu Chinese hospital。And then the information exchange functions of HL7/DICOM gateway are introduced, the various storage models in hospitals are analyses and a three storage model of this hospital is proposed。The image storage and communications systems (PACS), including its use of the module and function, county-level hospitals in the information technology and digital-building process, have a good application and reference。
