胰岛素受体对支气管哮喘患者外周血辅助性T淋巴细胞分化的影响
Effects of Insulin Receptor on T-helper Lymphocyte Differentiation in Asthmatic Patients in PBLs in Vitro

【摘要】 目的探讨胰岛素受体及其抗体对支气管哮喘患者外周血辅助性T淋巴细胞(Th细胞)分化的影响及其机制。方法选择支气管哮喘患者11例,所有受试者取外周静脉血分离提取淋巴细胞,将每位受试者的外周血淋巴细胞分为3组:一组加入胰岛素作为对照组;一组加入胰岛素及胰岛素受体α亚基抗体;另一组加入胰岛素及胰岛素受体β亚基抗体,3组均培养于无血清培养基。培养体系中加入佛波醇酯(PMA)和离子霉素激活细胞,布雷菲德菌素A(BFA)和莫能霉素阻止细胞因子向细胞外分泌。孵育6h后用流式细胞仪测定其γ-干扰素(IFN-γ)和白细胞介素-4(IL-4)的表达水平及IFN-γ阳性细胞百分比/IL-4阳性细胞百分比(Th1/Th2)的比值。结果1、与对照组比较,胰岛素受体α亚基抗体组IFN-γ阳性细胞的百分比显著增加(P<0.05),IL-4阳性细胞百分比无明显改变,Th1/Th2比值显著增加(P<0.05)。2、与对照组比较,胰岛素受体β亚基抗体组IFN-γ阳性细胞的百分比增加,但差异无统计学意义,IL-4阳性细胞百分比无明显改变,Th1/Th2比值显著增加(P<0.05)。3、与胰岛素受体α亚基抗体组比较,胰岛素受体β亚基抗体组IFN-γ阳性细胞的百分比无明显改变,IL-4阳性细胞百分比无明显改变,Th1/Th2比值无明显改变。结论胰岛素可通过胰岛素受体对Th淋巴细胞分化进行调节。胰岛素受体α亚基抗体与胰岛素受体β亚基抗体可以促进Th细胞向Th1细胞分化,增加表达Th1类因子的表达,在细胞分化水平上调节Th1/Th2平衡向Th1优势表达。
【Abstract】 Objective To investigate the effects of insulin receptor and insulin receptor antibody on T-helper lymphocyte differentiation in asthmatic patients in PBLs in vitro and it’s potential mechanism.Methods Blood samples were obtained from asthmatic patients(n=11). Separated peripheral blood lymphocytes(PBLs)were placed in serum free medium.Each PBLs sample was divided into insulin receptor(α-subunit) antibody treated group,insulin receptor(β-subunit)antibody treated group and the control group.All samples were stimulated with PMA and Ionomycin for 6h at 37℃in the presence of brefeldin-A and Monensin.The percentage of IFN-γ~+cells and the percentage of IL-4~+ cells and Thl/Th2 ratio were determined by Flow cytometry.Results 1.As compared with the control group,the percentage of IFN-γ~+ cells and Th1/Th2 ratio in the insulin receptor(α-subunit)antibody treated group showed a significantly increase(P<0.05),the percentage of IL-4~+ cells showed no significant differences between the two groups.2.The percentage of IFN-γ~+ cells and IL-4~+ cells were no statistically significant between the insulin receptor (β-subunit)antibody treated group and the control group,howerer,Th1/Th2 ratio showed a significantly increase in the insulin receptor(β-subunit)antibody treated group(P<0.05).3.Compared with the insulin receptor(α-subunit) antibody treated group,the percentage of IFN-γ~+cells and IL-4~+ cells and Th1/Th2 ratio were no statistically significant in the insulin receptor(β-subunit) antibody treated group.Conclusion Insulin receptor modulates the differentiation and phenotypes of T-helper lymphocyte.Insulin receptor(α-subunit)antibody and insulin receptor (β-subunit)antibody promoted the precursor Th cells(ThO)differentiate to Thl and increased the percentage of Th1 cells.It skews the Th1/Th2 balance to Th1.
