【摘要】 目的：1．观察头穴丛刺治疗急性脑梗死的疗效。2．分析适合急性脑梗死针灸疗效评价的量表。方法：将62例患者随机分为2组：头穴丛刺组、药物治疗组。观察各组Fugl-Meyer、NIHSS、SIAS、神经功能缺损评分、改良Barthel指数，评价两组的临床疗效。再分析各量表效度、信度、敏感度及评分时间。结果：1．疗效比较(1)两组均能提高FMA、SIAS评分，降低NIHSS、神经功能缺损评分，头穴丛刺对脑卒中后残疾的改善优于单纯药物治疗。(2)两组均能提高MBI评分，头穴丛刺对日常生活自理能力的改善优于单纯药物治疗。(3)头穴丛刺治疗急性脑梗死疗效确切，总有效率达93.33％，两组疗效差异显著(P＜0.01)，头穴丛刺疗效优于单纯药物治疗。2．量表分析(1)FMA、SIAS、神经功能缺损评分效度高，NIHSS效度差。(2)FMA、NIHSS、SIAS、神经功能缺损评分信度均较高。(3)FMA、NIHSS、SIAS、神经功能缺损评分敏感度均较高。(4)FMA评分时间最长，神经功能缺损评分时间最短，SIAS评分时间长于NIHSS。结论：1．头穴丛刺是一种有效的治疗急性脑梗死的方法。2．SIAS更适合评价针灸治疗急性脑梗死的疗效，建议在临床和多中心研究中推广应用。更多还原

【Abstract】 Objective:1. Observe a cluster needling of scalp acupuncture curative effect on acute cerebral infarction.2. Analyze the scales that are suitable for acute cerebral infarction acupuncture curative effect evaluation.Methods:62 patients were divided into 2 groups randomly , the treatment group were treated with cluster needling of scalp acupuncture plus medicine treatment and the comparison group were treated with medicine treatment. Observe each Fugl-meyer (FMA) , NIHSS, SIAS, nerve function defection and damage grade point, the improvement Barthel index number, evaluate two groups clinical curative effect .Analyze each scales validity, reliability, sensitivity and grade time.Results:1. Curative effect comparison(1) Two groups can increase FMA, SIAS, reduce NIHSS, the nerve function defection and damage grade point, and indicate cluster needling of scalp acupuncture is better than medicine treatment on impairment.(2) Two groups can increase MBI, indicate cluster needling of scalp acupuncture is better than medicine treatment on disability.(3)Cluster needling of scalp acupuncture is very well on acute cerebral infarction. Its efficient reach to 93.33%, differences of two sets is obvious (P<0.01).Cluster needling of scalp acupuncture is better than medicine treatment.2. Scales analysis (1) FMA, SIAS, the nerve function defection and damage grade point validity is high, the NIHSS validity is low.(2) FMA, NIHSS, SIAS, the nerve function defection and damage grade point reliability is all high.(3) FMA, NIHSS, SIAS, the nerve function defection and damage grade point sensitivity is high.(4)The FMA grade time is the longest; The nerve function defection and damage grade time is the shortest, the SIAS grade time is longer than NIHSS.Conclusion:1. The therapy of cluster needling of scalp acupuncture is an effect method to acute cerebral infarction.2. SIAS is more suitable for the evaluation acupuncture to cure acute cerebral infarction.更多还原
