【摘要】 目的研究大肠癌患者外周血鸟苷酸环化酶C的表达，并探讨其临床意义。方法应用逆转录聚合酶链反应(RT-PCR)技术，检测大肠癌患者术前外周血鸟苷酸环化酶C的表达，并将其与临床、病理各项指标进行统计学分析。结果8例非肠道肿瘤患者外周血中GC-C mRNA检测均为阴性，52例大肠癌患者外周血中GC-C mRNA检测阳性率为50％(26／52)，外周血GC-C mRNA的表达与肿瘤浸润深度(P＜0.05)、脉管瘤栓(P＜0.05)的差异有显著的统计学意义；而与外周血CEA值呈一致性(无统计差异学)。血液中GC-C mRNA的阳性表达率也随着临床分期的增加而增加，在29例CEA正常患者中GCC mRNA有9例阳性，其中伴有脉管瘤栓4例，伴有癌结节2例；而26例GCC mRNA阴性患者中CEA有6例升高，其中伴有脉管瘤栓2例。结论GC-C可在肠粘膜细胞、原发和转移性大肠癌细胞中特异性表达，但不在肠道以外的组织和肿瘤中表达，GC-C是大肠癌微转移的诊断、临床分期和术后随访监测有临床应用价值的新的血液学指标。更多还原

【Abstract】 Objective: To investigate the expression of GC-C in peripheral blood of colorectal cancer patients and to discuss it’s clinical significance.Methods: Use the RT- polymerase chain reaction to detect the GC-C mRNA in peripheral blood of colorectal cancer patients and analysis them with clinical and pathological indexes statistically.Results: The positive rates of GC-CmRNA in 52 cases were 48.08%(25/52), The positive rates of GC-C mRNA were associated with infiltration (P<0.05) and tumour embolus in vessel (P<0.05), the differences were significant . While to CEA level in peripheral blood (P>0.05). there were no statistical difference. The positive expression of GC-CmRNA increased with the clinical stage . In the 29 cases with normal CEA level, we found 9 cases with GC-CmRNA expression, which including 4 cases with tumour embolus in vessel and 2 cases with cancer tubercle . While in 26 cases with no GC-CmRNA expression we found 6 cases with high CEA level, which including 2 cases with tumour embolus in vessel.Conclusion: Guanylyl cyclase-C expressed selectively in cells derived from intestinal mucosa, including normal intestinal cells , primary and metastatic colorectal cancer cells , but not by extragastrointestinal tissues and tumors. Guanylyl cyclase-C was the valuable new clinical index in micrometastasis diagnosis, stage and postoperation of colorectal cancer.更多还原
