小柴胡汤治疗大鼠子宫内膜异位症的实验研究

【摘要】 目的: 通过建立大鼠子宫内膜异位症(内异症,Endometriosis,EMs)模型,研究中药小柴胡汤(Xiao Chaihu Tang)治疗大鼠内异症的效果及其可能机制。 方法: (1) 采用自身组织移植法建立大鼠内异症模型,将建模成功的大鼠随机分为4组,分别给予蒸馏水、小柴胡汤、丹那唑及小柴胡汤和丹那唑联合用药治疗4周。 (2) 通过观察治疗前后异位内膜体积的变化,定量分析异位内膜上皮高度的改变,评价各用药组内异症的治疗效果。 (3) 显微镜下观察在位内膜(Endometrium,EM)、异位内膜(Endometriotic,EOS)超微病理学改变。 (4) 免疫组织化学方法分析EM、EOS中环氧合酶—2(Cyclooxygenase-2,COX-2)及芳香化酶(Aromatase cytochrome P450,P450 arom)表达的改变,探讨小柴胡汤治疗内异症的可能机制。 结果: (1) 成功建立大鼠内异症模型。 (2) 用药治疗4周后,小柴胡汤组、丹那唑组及小柴胡汤和丹那唑联合用药组移植物体积明显减小(P<0.01)。 (3) 医学图像定量分析结果显示,小柴胡汤组、丹那唑组、联合用药组EOS上皮高度比对照组显著降低,各用药组内膜上皮的抑制程度不同,尤其以小柴胡汤和丹那唑联合用药抑制作用最为明显(P<0.01)。 (4)透射电镜下可见小柴胡汤组EOS腺上皮细胞出现凋亡特征,表现为细胞体积变小、核固缩、胞浆和核染色质凝集,细胞间出现凋亡小体,同时基质细胞中可见一些坏死细胞;丹那唑组EOS腺上皮细胞则呈点片状坏死灶,坏死细胞中的细胞器明显呈空泡状变性;小柴胡汤和丹那唑联合用药组腺上皮细胞可见凋亡小体和坏死病灶。 (5) 免疫组织化学染色结果显示:小柴胡汤组EOS中COX-2、P450 arom的表达低

【Abstract】 Objective:To investigate the effects of Xiao Chaihu Tang on endometriotic tissue and explore the probable mechanisms by means of the Endometriosis (EMs) rat models.Methods:(l)The rat EMs models established by surgical operation were divided into 4 groups.They were treated by water, Xiao Chaihu Tang,Danazol and Xiao Chaihu Tang plus Danazol for 4 weeks,respectively.(2)By observing and measuring EOS’volume before and past treatment and the height of endometrial epithelium(H.),to evaluate the therapic effect of each group.(3)Ultrastructural change in EM and EOS of each group were observed by elevtron microscopy.(4)COX-2 and P450 arom protein expression in EM and EOS of each group were observed and analyzed by immunohistochemistry method.Results:(l)The rat models of surgically induced endometriosis were established successfully.(2)Compared with the control group, the EOS’volume of the treatment groups was obvisously reduced(P<0.01) ,specially the Xiao Chaihu Tang plus Danazol group.(3)The diminution of the height in the treatment groups was more significant than the control group(P<0.01). The dimished rate of the height was obvious in Xiao Chaihu Tang plus Danazol group.(4)The glandular cell in the endometrial implant after therapy with Xiao Chaihu Tang showed characteristic feature of apoptosis(APO),which displayed the cell decreased in size, karyopyknosis, cytoplasmic and nuclear chromatin condensation,density increased and many apoptotic bodies among cell.Whereas some stromal cell displayed degeneration and necrosis. Many nectrotic cell and auto phagosome were commonly seen in the Danazol group.
