植入性大腿假肢设计改进与软组织固定罩材料制备及其组织学评价
Design Improvement and Histological Evaluation of Soft Tissue Fixation Flange for Implantable Leg Prosthesis

【摘要】 植入性假肢具有与健康人体接近的生物力学特征,是能完全恢复患者残肢功能新一代假肢的最理想的设计,但是其动态的生物密封是至今仍未解决的技术难题。除材料学因素外,从结构设计上降低植入性假肢经皮部位软组织/经皮器件界面上应力作用,为经皮部位的生物学密封提供有利条件,是值得关注和研究的问题。作为课题的一部分,本文的前期工作提出了在经皮密封部位皮下部件设计软组织固定罩结构,对周围组织起固定作用,降低经皮密封部位软组织在界面处的应力,为生物密封提供条件的设想,是有效和可行的。本文的工作重点是对软组织固定罩材料及表面处理对周围组织的生长的影响,通过动物实验和生物学评价进行验证。 本文选取了五种不同表面结构以及多种不同几何结构特征的样品植入动物皮下软组织,对植入后三个时间段的软组织的生长和表面结合情况进行观察和评价。结果表明纯钛表面等离子喷涂羟基磷灰石涂层处理的样品较好,其次为阳极氧化处理、酸碱处理及纯钛(抛光处理),最差的是超高分子量聚乙烯。孔间距对软组织在空中的生长无明显影响。样品植入60天之前,孔径和孔深度对软组织的生长有一定的影响,但90天时,这种差别已明显减小。因此,选取表面喷涂羟基磷灰石涂
【Abstract】 Osteointegrated artificial prosthesis has the close biological mechanical characters to healthy body, and is the optimal design of a new generation of artificial limb, which can completely recover the functions of patient’s disable limb, but its dynamic biological seal is still a technical problem up to now. Besides materials properties, there is a problem that deserves our attention and research, i.e. reducing the interfacial stress near seal and providing favorable condition to biological sealing between the percutaneuos device (PD) and the soft tissue of implantable artificial limb through structural design, it is proved efficient and feasible according to our prophase works. This paper focuses on the effects of the materials properties of soft tissue fixation flange and its surface modifications on growth of surrounding tissues, which was proved by animal experiments and biological evaluation.Five types of samples with different materials, different surface modifications, and construction dimension were implanted in the dorsal of animal. For three interesting period after implantation, relative histological evaluations of the tissue for samples were given. Results revealed that a more stable junction with soft tissue was formed for hydroxyapatite (HA)
