儿童皮肤癣病临床分离株的基因分型与药物敏感性分析
Analysis of Genotyping and Antifungal Susceptibilities of Clinical Isolates of Dermatophytes from Children

【摘要】 目的 ①应用RAPD技术对儿童皮肤癣菌病患者临床分离菌株进行基因分型,探讨菌株基因型与发病部位的相关性。②应用NCCLS推荐的M38-P方案对临床分离菌株进行抗真菌药物敏感性试验,试图发现基因型与抗真菌药物敏感性之间的关系。 方法 收集年龄12岁以下儿童皮肤癣病患者的毛发、皮屑、甲屑等标本进行真菌镜检及培养,对培养阳性的菌株,通过菌落形态及镜下形态进行菌种鉴定。用随机扩增DNA多态性(Random amplification of polymorphic DNA,RAPD)技术进行基因分型,参照文献报道使用以下4种随机引物①OPAA11:5’—ACCCGACCTG—3’②OPD18:5’—GGTGACGCAG—3’③5’-GGTGCGGGAA-3’④5’-CCCGTCAGCA-3’。抗真菌药物敏感性试验采用微量液体稀释法(参照M38-P方案),包括8种抗真菌药物:氟康唑(FCZ),伊曲康唑(ICZ),特比奈芬(TBF),酮康唑(KCZ),利拉奈酯(Liranaftate),布替奈芬(Butenafine),益康唑(ECZ),联苯苄唑(bifonazole)。 结果 ①共分离出皮肤癣菌35株:其中红色毛癣菌25株(占71.4%),包括体癣12株,甲癣6株,足癣4株,股癣3株;犬小孢子菌4株(占11.4%),包括头癣2株,体癣2株;须癣毛癣菌4株(占11.4%),包括头癣2株、体癣2株;断发毛癣菌2株(占5.8%)均分离自头癣。基因分型结果显示:引物OPAA11和引物OPD18扩增的带型稳定、清晰,符合RAPD基因分型的要求,用于本次试验的结果分析。引物OPAA11的RAPD分型结果显示:25株红色毛癣菌大部分菌株基因型相同,体癣、甲癣、股癣均以H1a型为主,而足癣的基因型以H2a型为主:其余菌株多态性不明显。引物OPD18与引物OPAA11的分型结果大部分一致,也有少部分菌株出现分型不一致的情况。②红色毛癣菌基因型与抗真菌药物敏感性试验结果经Kruskal-Wallis秩和检验表明:基因型H1a和H2a对氟康唑、
【Abstract】 Objective ①To investigate the DNA type of clinical isolates of dermatophytes from children using RAPD, and to study the relationship between genotypes and the sites of infection of dermatophytes.② A modified M38 - P scheme was used in evaluating the antifungal susceptibility of clinical isolates, and to investigate the relationship between DNA type and antifungal susceptibility.Methods To collect the specimens of skin and nail scrapings ,hairs from dermatophytosis under twelve years. Microscopic examination and fungal cultures were made and all strains were morphologically identified. The genotypical analysis was performed using the random amplification of polymorphic DNA (RAPD) method, the primers used were from literatures, the sequences of the four primers were 5’-ACCCGACCTG-3’, 5’-GGTGACGCAG-3’, 5’-GGTGCGGGAA-3’, 5’-CCCGTCAGCA-3’,respectively. The antifungal susceptibility study was conducted using the microdilution test , eight antifungal agents including Fluconazole , Itraconazole, Terbinafine, Ketoconazole, Liranaftate. Butenafine. Econazole and Bifonazole were tested..Results ① Thirty-five dermatophytes were isolated from children:25 Trichophyton rubrum (71.4% ) was isolated from 12 cases of tinea corporis, 6 cases of onychomycosis,4 cases of tinea pedis and 3 cases of tinea cruris; 4 Trichophyton mentagrophytes(11.4%) and 4 Microsporum canis.{11.4%) were isolated from 2 cases of tinea capitis and 2 cases of tinea corporis respectively;2 Trichophyton tousurans
