硫酸镁作为口服对比剂在小肠疾病MRI检查中的价值

Value of Magnesium Sulfate as an Oral Contrast Medium for MRI Inspection of Small Intestine Diseases

【摘要】 目的:研究以硫酸镁为对比剂在小肠MRI检查中的应用及其在小肠病变诊断中的价值,与小肠气钡双对比检查对比,进一步探讨MRI在小肠病变诊断中的地位及发展前景。 材料与方法: 一、对比剂的选择: 1、以水为参照物,将不同浓度的硫酸镁置于磁共振扫描仪内进行同一序列的扫描,比较所选不同浓度硫酸镁对比剂对成像信号的影响,选择成像信号清楚的对比剂浓度用于小肠MRI检查。 2、在相同情况下选择志愿者以水和一定浓度的硫酸镁进行胃肠道磁共振检查,对其检查结果进行分析、综合,比较其是否适合作为小肠MRI对比剂。 二、临床应用:对于临床疑诊有小肠病变的31例病人按双盲法分别进行气钡双对比检查与小肠MRI检查,按双盲法分别由普放医师与MRI科医师独立操作,独立作出诊断,最后对检查结果进行汇总分析,并与临床治疗效果、手术病理结果和活检病理结果进行对比,观察不同疾病的MRI表现,患者耐受性、不良反应及并发症情况。 结果: 一、对比剂的选择: 不同浓度的硫酸镁产生不同的信号,清晰度不同,通过测试不同浓度硫酸镁,选择2.5%硫酸镁作为对比剂,2.5%硫酸镁渗透压在小肠平均扩张度、小肠扩张度、图像质量方面均优于水。 二、临床应用: 31例患者进行小肠磁共振成像检查均获成功,大多数患者可以耐受,小肠磁共振成像诊断结果为:克罗恩病(Crohn病)18例,十二指肠肿瘤1例,小肠肿瘤5例,

【Abstract】 Objective:To explore the value of the magnesium sulfate as an oral contrast medium for small intestine magnetic resonance imaging (MRI) and the diagnosis of the small intestine diseases and to discuss the status and the development prospect of the MRI inspection of the small intestine diseases comparing with the double contrast barium. Materials and Methods:1. the selection of the Contrast MediumThe water, as the frame of reference, and the magnesium sulfate with different concentrations were scanned with the same identical MR sequence. After study and comparison of the signals of magnesium sulfate with different concentrations, the ideal concentration of magnesium sulfate was defined for small bowel MRI. On the same situation, volunteers were examined with MRI after drinking water or the magnesium sulfate with some certain concentration at different time and the result was analyzed.2. Clinical practice:Thirty-one patients with suspect of small intestine disease were studied by MRI and the double contrast barium to analyze the findings of the disease. The diagnosis was independently made by MRI doctors and radiologists. The MRI findings in 31 cases with small intestinal diseases were compared with findings of conventional intestinal enteroclysis and the results of pathology. The MRI manifestations of the different disease, the patient tolerance, the side affection and the complication, the clinical practice value of the magnesium sulfate as an oral contrast medium for small intestine magnetic resonance imaging were observed.
