学龄期唐氏综合征儿童交流行为的研究

【摘要】 目前国外关于唐氏综合征儿童交流行为的研究比较丰富,使得人们能够更清晰地认识唐氏综合征,也为唐氏综合征患者的康复和生活质量的提高提供了理论指导。国内在该领域的研究非常少见。唐氏综合征是最常见的一种基因异常疾病,国内已有的研究大多将DS儿童和其他智力落后儿童作为一个整体来考察,削弱了研究成果对实际教学和训练的指导意义和应用价值。因此国内开展唐氏综合征儿童交流行为的研究非常必要。 本研究以DS儿童与家长在半结构化情境中的交流过程,记录他们在此过程中的言语和非言语交流行为,运用INCA-A对这些行为进行编码,然后利用CLAN对这些行为进行分析。 研究发现,言语行为是DS儿童与家长进行交流的主要方式,DS儿童的非言语交流行为对交流的顺利进行起着重要的作用。随着智龄的增长,DS儿童的交流能力有所提高;DS儿童运用交流行为的能力随着智龄的增长而发展,交流行为得运用越来越灵活。智力水平对言语行为的影响较大,对非言语行为的影响较小;在社会交往倾向性、言语行为类型和言语流畅性三个方面,智力水平的影响程度不同;DS男生和女生之间的交流行为不存在显著性差异。 研究发现,DS儿童和家长交流过程中的社会交往倾向类型主要有“谈论目前共同关注的事物、事件和活动(DJF)”以及“商议即刻进行的活动(NIA)”。在与家长的交流过程中,DS儿童一般处于被动的地位,他们比较顺从家长提出的建议和要求。DS儿童和家长之间的言语行为互相影响,存在相关。 研究者提出在对DS儿童的教育和训练中,应该增加儿童与他人的交流机会,并建议以智龄作为教育和训练计划的参考。

【Abstract】 Since there are a large number of researches on communicative acts in children with Down syndrome, the knowledge on Down syndrome has been improved. Therefore, the rehabilitation of children with Down syndrome has been strongly supported on the basis of these researches. Few researches in this field have been developed in our country. Since the current related research has been done putting Down syndrome in the group of children with mental retardation, the effects of the results are limited in clinical use.This research was conducted in a semi-structured play context which the children with Down syndrome and their main care-givers take part in. The procedures of their communication were video-recorded. The speech acts and nonverbal -communication acts during their communication were transcribed. As effective instruments, INCA-A and CLAN were employed to code and analyze these communicative acts. On the basis of the analysis, some conclusions were drawn.Speech acts are the main communicative medium used by the children and main care-givers during their communication, while nonverbal-communication acts play an important role in making the child understood. The communicative acts in children with Down syndrome do develop while their mental-age grows. The cognitive levels of the children with Down syndrome influence their communicative performance, and the influence differs within the levels of Social Interchange, Speech Acts and Pragmatic Flexibility. The cognitive levels make more impact on the performance of speech acts than the nonverbal-communication acts. There is no significant difference between the boys and girls with Down syndrome generally, and the only difference occurred in TXT, which is one of the social interchange types.Discussing a joint focus attention (DJF) and Negotiating the immediate activity (NIA) are the main social interchange types during the communication between the
