针刺运动区上2/5对大鼠坐骨神经损伤修复的实验研究

【摘要】 目的:通过观察头针在不同时间段对大鼠坐骨神经损伤再生修复的影响,旨为进一步揭示头针在不同的时间段对大鼠神经损伤再生修复的作用机制,为临床治疗周围神经损伤提供理论依据。 方法:本实验以Wistar雄性大鼠为受试对象,采用坐骨神经钳夹伤的周围神经损伤模型,将180只大鼠随机分为:(1)针刺运动区上2/5组(头针组);(2)针刺运动区上2/5+局部针刺组(头体针组);(3)局部针刺组(体针组);(4)药物组;(5)模型组;(6)假手术组,每组又分为三个时间段(7d、14d、28d)。采用HE染色观察损伤的神经及相应节段脊髓组织形态学变化;从电生理方面动态观察其功能恢复状况;应用免疫组织化学技术检测坐骨神经损伤神经元中IGF、FGF的表达。 结果:1、针刺各组与药物组MNCV均有不同程度提高,治疗组之间有显著性差异(P<0.05)。2、头针和体针均能减轻坐骨神经损后神经纤维病变程度作用优于药物组,可促进轴突再生,其中头体针结合效果最佳。3、头针组IGF、FGF阳性表达显著,治疗四组之间有显著性差异(P<0.01)。 结论:1、头针能促进坐骨神经损伤大鼠运动神经传导速度(MNCV)的恢复。2、头针能减轻大鼠坐骨神经损伤后神经和相应脊髓节段神经元肿胀、变性,减少神经元死亡,促进轴突再生。3、头针可以抑制坐骨神经损伤大鼠神经元中IGF、FGF表达水平的降低,促进其恢复,从而保护相应神经元,促进神经再生。4、头体针结合效果最佳,能更好的促进损伤神经的再生和功能恢复。

【Abstract】 Objective: Observing the influence of the head acupuncture to the regeneration and repair of the sciatic nerve injury in rats at different time, unveils the active mechanism of the regeneration of the sciatic nerve injury in rats at different time, offers the theoretical basement clinically.Methods: The Wistar male rats are the testers in this test. Adopts the model of peripheral nerve injury by nipping the sciatic nerve, makes these 180 rats into six groups: (1) the group of acupuncturing the upper 2/5 of motor zone (the head acupuncture group);(2) the group of acupuncturing the upper 2/5 of motor zone+ the local (the head and body acupuncture group);(3) the local acupuncture group (the body group);(4) the drug group;(5)the model group;(6)the pseudo operation group. Every group is divided into three time segments (7d, 14d, and 28d). Observes the change in morphologic of the injured nerve and the corresponding spinal segment with HE staining;looks into the recovery condition of the function dynamically in electrophysiology;examines the expression of the IGF and FGF in the injured sciatic nerve cells with immunohistochemistry.Results:1.The MNCV increases at different extents in these acupuncture groups and the drug group, the comparison between treatment groups showed significant difference (P<0.05) .2. Not only the head acupuncture and body acupuncture can relieve the pathological changes degree of nerve fiber resulted in peripheral nerve injury, accelerate the regeneration of the nerves, and the effect is superior to drug group. The combination of the head acupuncture and body acupuncture is second to none.3. The expression level of IGF and FGF of head acupuncture group is significant. The comparison between treatment groups showed significant difference (P<0.01) .Conclusions:1. The head acupuncture can promote the recovery of the motor nerve conductive velocity (MNCV) of the sciatic nerve injury in rats.2. The head acupuncture can relieve the swell and the denaturalization of the nerves and the nerve cells which are located in the corresponding spinal segment, cut down the death of the nerve cells and accelerate the regeneration of the axon after the sciatic nerve of the rat is injured.3. The head acupuncture can restrain the decreasing of the expression level of IGF and FGF in the nerve cells of rats whose sciatic nerves are injured, makes them recover, and anymore protects the nerve cells, accelerates the regeneration of the nerves.4. The combination of the head acupuncture and body acupuncture is second to none. As to promoting the regeneration and repair of nerve injury, it can do better.
