清脑宣窍滴丸对脑缺血损伤的保护作用及其机制初探

【摘要】 脑血管疾病具有发病率高、致残率高、死亡率高和复发率高等特点,严重威胁人类健康,给个人、家庭和社会带来巨大的精神压力和沉重的经济负担。据流行病学调查得知,在我国脑血管病的死亡率从1997年开始已跃居各种疾病之首。而在脑血管疾病中缺血性脑血管病又占脑血管病的多数(约占其总数的55~80%),因此深入探讨缺血性脑血管病的发病机制,明确缺血病变过程中的主要环节,具有十分重要的医学价值。清脑宣窍方是王永炎院士的临床经验方,由三七、栀子、冰片组成,组方遵循中医药理论,功效为醒神开窍、解毒通络,在治疗缺血性脑中风急性期和恢复期早期方面具有很好的疗效。本校中药化学教研室采用大孔树脂分离富集本方的有效部位,运用现代制剂工艺改革剂型制成清脑宣窍滴丸(QNXQD),毒副作用小、用药量少,是很具前景的治疗缺血性脑血管病的中药。本研究结合目前脑缺血研究的进展,利用生理、生化、免疫组织化学等方法,从整体、组织及分子水平观察清脑宣窍滴丸对脑缺血损伤的保护作用及可能的作用环节。1清脑宣窍滴丸有效部位制取工艺合理性的药效学研究1.1清脑宣窍滴丸上大孔树脂前提取物及上大孔树脂后有效部位对FeCl3致大鼠大脑中动脉血栓模型(MCAT)神经症状及脑梗塞范围的影响采用三氯化铁致大鼠大脑中动脉血栓模型(MCAT),通过测定大鼠神经症状评分及脑梗塞范围,比较清脑宣窍滴丸上大孔树脂前提取物和上大孔树脂后有效部位对MCAT大鼠的保护作用。结果显示,清脑宣窍滴丸过大孔树脂柱前提取物及过大孔树脂后有效部位各剂量组可不同程度改善MCAT模型大鼠术后6、24小时神经症状;清脑宣窍滴丸过大孔树脂柱前提取物248.4mg/kg、496.8mg/kg(相当于生药量0.9g/kg、1.8g/kg),过大孔树脂后有效部位90mg/kg、180mg/kg(相当于生药量0.9g/kg、1.8g/kg)均可使MCAT大鼠脑梗塞程度有明显减轻;但清脑宣窍滴丸过大孔树脂柱前提取物与上大孔树脂后有效部位之间相比作用并无显著差异。1.2清脑宣窍滴丸上大孔树脂前提取物及上大孔树脂后有效部位对拟气虚血瘀小鼠模型断头张口呼吸时间的影响采用8℃冷水负重游泳拟气虚血瘀模型,通过测定冷水负重游泳5分钟后
【Abstract】 Cerebrovascular, well known for its high incidence, high death rate, high relapse rate and high mutilation rate, is one of the most serious threat to the health of human being, which brought serious burden to the patients, families and society. According to the epidemiological study, the death rate of cerebrovascular disease has become the first of all the diseases since 1997. The incidence of ischemic cerebrovascular disease is higher than hemorrhagic cerebrovascular disease. So deeper research about the mechanism of ischemic cerebrovascular disease and the main points of the pathological changes has extremely important significance in modern medical.Qingnaoxuanqiao Compound Prescription is an experienced Compound Prescription in clinical by Academician Wang Yongyan, which consists Fructus Gardeniae, Radix notoginseng and Borneolum syntheticum. It follows the principle of traditional Chinese Medicine and has a good effect in the treatment of acute issue and early convalescence period of ischemic brain apoplexy. The phytochemistry laboratory got this effective fraction using macrous absorptive resins to assemble the active component. It is a prospective drug in curing the ischemic cerebrovascular disease which has less toxicity, less side effects and small dosage.In the dissertation, the effects of Qing Nao Xuan Qiao Dropping pill(QNXQD) were investigated in vivo and in vitro respectively on tissue, cellular and molecular levels depending on the current researches on mechanism of ischemic cerebrovascular disease.
