地塞米松对实验性ARDS时细胞因子变化影响的研究

【摘要】 目的:观察地塞米松在不同时间干预对兔油酸型急性呼吸窘迫综台征(ARDS)血清TNF-α、IL-8及IL-10的影响,探讨糖皮质激素治疗ARDS的理论基础。方法:将40只大白兔随机分成正常对照组(n=8),ARDS模型组(n=8)和地塞米松治疗组(n=24)。经耳缘静脉注射油酸复制兔ARDS模型,治疗组再分成三组(每组n=8)分别于即刻、90min、120min注射地塞米松治疗,用放射免疫及酶联免疫吸附试验测定血清中TNF-α、IL-8及IL-10水平。结果:ARDS组兔血清TNF-α水平高于对照组(P<0.01);IL-8水平与对照组相比无明显变化(P>0.05);而IL-10水平低于对照组(P<0.01)。经地塞米松治疗后,TNF-α、IL-8水平降低,IL-10水平升高。90min检测TNF-α水平与ARDS组相比有统计学差异(P<0.01),120min检测TNF-α、IL-8及IL-10水平与ARDS组相比均有统计学差异(P<0.01)。并且,不同时间应用地塞米松治疗后血清TNF-α、IL-8及IL-10水平有相应的不同变化,以120min用药变化显著。结论:TNF-α、IL-8及IL-10在兔油酸型ARDS炎症过程中可能起重要作用。地塞米松可通过干预TNF-α、IL-8及IL-10分泌而减轻肺损伤。同时,地塞米松干预时间与炎症的发生、发展存在一定关系。

【Abstract】 Objective: To observe the effects of dexamethasone intervened at different time on TNF-α、 IL-8、 IL-10 in serum of rabbits with ARDS induced by oleic acid, and to explore the theoretical foundation of glucocorticoids treatmented ARDS. Methods: The rabbits were randomly divided into the normal control group (n=8), ARDS group (n=8) and dexamethasone-treated group(n=24). The animal model of ARDS was established by injected oleic acid to rabbits’veins. The dexamethasone-treated group was divided into three group, and injuected dexamethasone at different times. Using radioimmunoassay and enzy linked immunosor-beng assay (ELISA)measured the levels of TNF-α, IL-8, IL-10 in serum. Results: In ARDS group the level of TNF-α in serum were significantly higher than those of control group (P <0.01), IL-8 in serum were not obvious variation (P >0.05) and IL-10 in serum were significantly lower than those control group (P <0.01).After dexamethasone injected ,the levels of TNF-α, IL-8 in serum were lower and IL-10 in serum higher.The levels of TNF-αin serum was significantly higher than those of ARDS group (P <0.01) at 90min;the levels of TNF-α, IL-8 and IL-10 in serum were obvious variation than those of ARDS group(P <0.01) at 120min. There were different variation with TNF-α, IL-8, IL-10 in serum when dexamethasone was given at different times.These variation was more obviousness at 120 min. Conclusion: TNF-α, IL-8, IL-10 may playimportant role in the inflammatory process of oleic acid ARDS in rabbits, dexamethasone may relieve lung injuries by intervene discharging of TNF-ax IL-8, IL-10. At the same time, there were possible relation at the progress of inflammation with the intervene time of dexamethasone.
