椎动脉型颈椎病新模型研究

【摘要】 目的:利用瘀血阻络的方法,建立一种新的椎动脉型颈椎病(CSA)动物模型,并探讨CSA的发病机理及药物治疗机制。 方法:选择50只健康日本大耳白家兔,雌雄各半,先随机分为2组,正常对照组8只,拟造CSA模型组42只,2组间在TCD、ENG、ABR、等方面无差异。将制备好的异体血液9ml分别注入模型组家兔左侧颈椎软组织造模,于造模后第2周对42只日本大耳白家兔分别测定TCD、ENG、ABR、VsEPs、小脑浦肯野氏凋亡细胞计数及前庭血流,确定模型出现。第3周随机将其分为CSA模型组、“止眩”治疗组和西比灵治疗组。每次前庭血流检测后随机处死5~7只动物,取家兔小脑和颈椎,进行病理学观测。 结果:经TCD、椎动脉X线数字减影检查证实CSA动物模型2、4、6周组基底动脉(BA)、左侧椎动脉(LVA)、右侧椎动脉(RVA)血流速度明显减慢;左侧椎动脉相对右侧椎动脉全程细小,有多个节段的迂曲。经ENG、ABR、VsEPs、前庭血流及小脑浦肯野氏凋亡细胞计数检查证实CSA动物模型2、4、6周组出现脑干功能障碍。胃肠灌注“止眩颗粒”、西比灵对动物模型脑干神经功能、BA、LVA、RVA的血供具有明显恢复作用。 结论:利用局部注射血液导致瘀血阻络的方法形成椎动脉型颈椎病的模型是成功的和切实可行的,方法简便、安全、可操作性强;具有维持时间较长,可自行消失的特点,完全符合实验动物模型的要求;提示CSA发病可直接由颈部力学平衡失常所导致,交感神经激惹学说及机械压迫学说也参与CSA的形成。

【Abstract】 Objective:To Set up a kind of new animal’s model of Vertebral artery type of Cervical Spondylosis, according to sludged blood hind the method in place with a net;To explore mechanism of the cause of CSA, and the mechanism of treating by drugs.Method : 50 health rabbit (The male and female are half and half) were divided into 2 groups randomly, 8 rabbit as a group is normally, 42 rabbit as another group plan to make CSA model, there is no difference in TCD, ENG, ABR ,etc. among 2 groups, inject into model group rabbit left side neck pheasant’s soft tissue by sludged blood 9ml, 42 rabbit were measured by TCD, ENG, ABR, VsEPs, cerebellum Purkinje die cell count From the 2th weeks after injecting, Confirm the model appears. 50 health rabbit were divided into 3 groups randomly From the 3th weeks after injecting, Put 5 animals to death at random after Laser Doppler Flowmetry Electroacupuncture is measured each time, fetch rabbifs cerebellum and cervical vertebra, carry on pathology to observe.Result: it verifies CSA animal’s model that From the 2th,4th,6th weeks after injecting, the speed of blood flow of BA, LVA, RVA obviously slow down by TCD, compared with The right side of artery of vertebra, The left side of artery of vertebra whole journey is tiny, tortuous that there are a lot of segments. it verifies CSA animal’s model that From the 2th,4th,6th weeks after injecting, the function obstacle of brain stem were found out by ENG, ABR, VsEPs, Laser Doppler Flowmetry, cerebellum Purkinje die cell count. the sibeline group was given to affuse stomach , The function of brain stem and the supplication blood obviously resume by given to affuse stomach that Jingshu particle and Xibiling.Conclusion: The method is successful and feasible by Utilizing part inject blood cause stasis of blood, held in place with a net to hinder form vertebra artery type model, the method is simple and convenient, the security, effectiveness;The model have the characteristics that keep time relatively long, disappear by itself, totally accord with the demands for model of animal. It reveal that the cause of CSA can directly by mechanics balanced, the theory of sympathetic nerve by Irritabled and the theory of machinery oppress participate in forming of CSA.
