肠易激综合征腹泻型临床中医分型

【摘要】 目的 对成都及其附近地区肠易激综合征(IBS)腹泻型患者临床证候调查及影响因素分析,以探知其症状、中医证候出现概率及相关因素对证候的影响,探讨肠易激综合征中医发病机理和治疗。 方法 通过对成都地区腹泻型IBS患者行问卷调查,了解性别、年龄、职业、文化程度、既往肠道感染史、女性月经史、精神因素、应激事件刺激及躯体症状等因素,将所得计数资料进行统计分析,按统一标准进行中医症候分型。 结果 1)IBS 女性患病率高于男性;患病率无明显年龄差异趋势;较高文化程度群体患病率高于较低文化程度的群体;腹痛及腹部不适是患者发生频率较高的症状;男性与女性在常见症状的发生频率上差异无显著性,但精神心理症状的发生频率上,女性应激事件发生率及抑郁状态发生率高于男性;肠道感染史、受凉、生冷饮食、辛辣饮食均为可能导致IBS发病的较危险因素;肠外症状中较常出现的为头痛、腰背痛、疲乏及功能性消化不良。 2) 进行中医症候分析,将其分为肝脾不和、脾虚气亏、大肠湿热、脾肾阳虚四型,肝脾不和、脾虚气亏两证的发生率明显高于其它两证。 结论 IBS是成都及其附近地区的常见功能性疾病,易为情志所困,中医治疗有巨大潜力。更多还原

【Abstract】 Objective To investigate the clinical symptoms of irritable bowel syndrome (IBS) diarrhea and analyze its effect factors in Chengdu and its nearby regions, knew the appearance rate of the symptoms and Zheng patterns of TCM and the influence of relevance factors, and researched the pathogenesis and therapy of TCM.Methods Using a questionnaire on the patients who suffered IBS diarrhea in Chengdu and its nearby region, including sex, age, occupation, educational degree, bowel infection history, female menstruation history, spirit factors , life stress and body symptoms, analyzed the datum investigated by statistics. According to the consentaneous diagnostic criteria, divided the disease into Zheng patterns of TCM。 Results 1) Female was predominated in IBS, compared with male. With the increase of age , the prevalence rate of IBS did not rise. The higher educational degree community had a higher morbidity of IBS than the community of the lower educational degree. Abdominal pain and discomfort are the symptoms, which more happened. Male and female did not show the distinction on the common symptoms. But female with IBS seemed to have a higher tendency of abnormal personalities than male, especially in life stress and depression.
