养阴清热口服液对鼻咽癌放疗所致唾液腺损伤影响的临床研究

【摘要】 目的:研究养阴清热口服液影响鼻咽癌放疗所致唾液腺损伤的作用机制,为临床应用养阴清热口服液提供理论依据。 方法:选择鼻咽癌首程放疗患者50例,按随机数字表法分为观察组(中药加放疗)25例和对照组(单纯放疗)25例。分别在放疗前、放疗至12Gy、40.8Gy、76.8Gy(即结束)时测唾液流速、唾液淀粉酶分泌速度,放射性核素技术观察腮腺、颌下腺的排泌(泌碍)功能,放疗期间记录患者的一般状况:唾液腺急性损伤、口咽反应、青紫舌情况。 结果:随着放疗剂量的增加,两组唾液流速与淀粉酶分泌速度下降,腮腺、颌下腺的排泌功能下降,唾液腺急性损伤、口咽反应加重,青紫舌比例增多。但观察组各项检测数值下降程度均比对照组轻,具有显著性差异(唾液流速与淀粉酶分泌速度:12Gy时P<0.05,40.8Gy、76.8Gy时P<0.01;腮腺、颌下腺的排泌功能:12Gy、40.8Gy、76.8Gy时P<0.05),患者的唾液腺急性损伤程度,口咽反应程度,青紫舌比例均比对照组低,具有显著性差异(P<0.05)。 结论:养阴清热口服液可能通过增加唾液流速、淀粉酶分泌速度;增大腮腺、颌下腺的泌碍率从而减轻了唾液腺急性损伤程度、减轻了口咽反应、减少了青紫舌比例,在一定程度上能够保护唾液腺,提高患者的生活质量。

【Abstract】 Objective: To observe the effect of Chinese herbal medicine on the salivary glands of patients with Nasopharyngeal cancer during radiotherapy (RT).Methods: Fifty cases with NPC were randomly divided into observation group treated with RT plus Chinese herbal Medicine and control group treated with RT alone.The symptom of patients,such as the severity of salivary glands damnification, the mouth reaction and qingze tongue was noted . And at the same time, the flow speed and amylase content of salivary, the rate of secreting 99mTcO4- were measured before and during the RT.Results: with the dose of RT increasing,the flow speed and amylase content of salivary declined, the same results were mersured with radioisope Scintiphoto graphy.But the declining degree in observation group was significantly more falttend than that of the control group (flow speed and amylase content of salivary, 12Gy, P<0.05,40.8Gy、 76.8Gy, P<0.01; the rate of secreting 99mTcO4- , 12Gy、 40.8Gy、 76.8Gy, P<0.05) .The severity of salivary glands damnification in observation group was lighter than that of the control group (P<0.05) ,the same results were observed in the mouth reaction and qingze tongue.Conclusion: Chinese herbal Medicine used in this study was effective to increase the flow speed, amylase content and the rate of secreting 99mTcO4- and so protect the salivary glands of patients with NPC during RT.
