海狸鼠尾部肌腱制作医用可吸收缝合线的研究

【摘要】 医用可吸收手术缝合线的研究是生物医用高分子研究中最活跃的领域之一,已广泛应用于医学和动物医学的外科手术中。本研究的目的就是要研制一种新型的,具有良好的生物相容性、较低的抗原性和优良的降解、吸收性能的手术缝合线。研究的主要内容有: 1.肌腱缝合线的制作及物理性能的研究:采用海狸鼠尾部肌腱经过脱脂、脱水、两次固定等一系列加工过程而制成缝合线,然后测算它的长度、直径及抗拉强度。结果发现,55%的肌腱长度在20 cm以上,80%多的肌腱直径集中在0.30~0.59 mm之间;用适当浓度的戊二醛溶液处理的肌腱缝合线抗拉强度最大,达275.43 N/mm,此强度完全符合伤口愈合的要求。 2.肌腱缝合线细胞毒性的研究:将它与鸡胚成纤维细胞共同体外培养,观察其对细胞形态和增殖率的影响。结果表明,这种缝合线对细胞形态无影响,对细胞增殖具有一定的促进作用,细胞相容性好,符合医用缝合线的要求。 3.生物相容性的研究:对肌腱缝合线进行了以下试验:急性毒性试验、溶血试验、热原试验、皮肤刺激试验,根据标准对试验数据进行分析和评估,结果显示肌腱缝合线的生物相容性试验结果均为阴性,从而说明了这种缝合线的生物相容性好,是理想的医用生物材料。 4.抗原性的研究:将大量肌腱缝合线溶液输入家兔耳静脉内,观察家兔的免疫反应,并分别取肌腱缝合线溶液、天然兔抗原、天然牛抗原皮下多点、腹腔、肌肉注射途径常规免疫家兔,来研究这三种抗原的免疫性强弱.结果发现,肌腱缝合线溶液的免疫反应性很弱。 5.植入后反应及降解性能、吸收性能的研究:用肌腱缝合线缝合家兔胃、小肠壁,发现缝合后家兔无体温升高现象,呼吸、脉搏正常,白细胞记数正常,病理检查正常。肌腱缝合线在家兔体内14天时张力降解为原来的60%左右,此时直径开始逐渐变细,35天时,肌腱缝合线已难测量其直径,60天后成粘糊状,用摄子轻夹缝线即碎裂,120天后肌腱缝合线完全降解吸收,此降解和吸收速度完全符合医用可吸收缝合线的要求。 由此可知,用海狸鼠尾部肌腱可以制作成一种新型的可吸收缝合线,此缝合线具有良好的生物相容性,较低的抗原性,降解、吸收性能优良的特点,是一种理想的可吸收手术缝合线。

【Abstract】 The study on the absorbable suture is one of the most active domains on biomedicine macromolecule research. They had been used in medicine and veterinary widely. The purpose of this research is to manufacture a new style suture which has excellent biocompatibility, lower antigenicity, good degradability and absorbility. The following are the main content:1. the production of tendon suture and studies on physical properties of it: tendon suture was produced by fat and water removal tendon that was double fixing, etc. Then the length and diameter of sutures were measured and the tensile strength was calculated. Results showed that the length of 55 percent tendon was more than 20 cm and the diameter of 80 percent tendon was around 0.30—0.59 mm. The tensile strength of tendon sutures which were crosslinked by 2% concentration glutaraldehyde is the strongest. It is 275.43 N/mm that is ideal for the demand of wound healing.2. research on the cytotoxicity of the tendon suture: the suture materials and chicken embryo’s fibroblasts were cultivated in vitro together. And the effect of the shape and proliferation of cells were observed. Results showed that this new style suture hadn’t negative effect on cell’s formation but active effect on proliferation of cells. So it has an excellent biocompatibility and is suitable for making medical suture.3. studies on the compatibility of the tendon suture: some experiments were done based on the cell toxicity test, they are urgent toxicity, pyrogenic reaction, skin stimulated and hemolytic test. Then their test datas were analyzed and evaluated according to ISO. These experiments had the negative outcomes. It showed that the tendon suture had good biocompatibility, and it is an ideal biology material for medicine.4. studies on antigenicity of tendon suture: a large amount of tendon suture solution were injected into rabbits by vein immunizing to study their immunological responses and reactions. And tendon suture solution, bovine’s Hb and rabbit’s Hb were infused into rabbits by subcutaneous , abdominal cavity and muscle immunizing. It was intended to study the degrees of antigenic activity. Inference can be drawn that there would unlikely emerge accidents in rabbits after infusion tendon suture solution.5. the reactions after implantation and research on the degradation property
