血竭对心肌缺血损伤保护作用研究

Protective Effects of Dragon’s Blood on Myocardial Ischemia

【摘要】 血竭(Dragon’s Blood,DB),又称为龙血竭,为百合科植物剑叶龙血树含树脂木材的乙醇提取物,具有活血散淤、止痛、止血、生肌敛疮等功效;内服主治淤血肿痛、经闭、痛经,外用主治外伤出血、溃疡不愈、痔漏肿痛,为著名的活血化淤中药之一。血竭中含有多种化合物,不同提取工艺获得的血竭因其纯度及化合物种类不同而功效有所不同。本实验改良血竭的提取工艺获得血竭制剂,选用杂种犬采用结扎冠状动脉前降支(LAD)复制急性心肌缺血模型,血竭十二指肠造口给药,观察比较三种剂量30mg/kg、60mg/kg、120mg/kg对心肌缺血犬血流动力学等各项指标的影响,并初步探讨了其作用机制。实验结果表明:血竭能明显改善心肌缺血动物血流动力学,降低心肌缺血动物心肌缺血程度(∑-ST),减少心肌缺血范围(N-ST)及心肌梗死面积,降低血清谷草转氨酶(AST)、肌酸磷酸激酶(CPK)和乳酸脱氢酶(LDH)的含量,明显降低血清丙二醛(MDA)含量,同时增加超氧化物歧化酶(SOD)活力。三个不同剂量组比较,大剂量组(120mg/kg)优于其他各组。证明血竭具有抗心肌缺血的作用,其机制与减轻自由基损伤有关。

【Abstract】 Ischemic heart disease (IHD) is a kind of myocardialdamage caused by the imbalance of myocardial blood supplyand demand, which comes from coronal blood circulationchanges. It includes myocardial ischemia which in turn,comes about due to acute and chromic diminished bloodsupply.Its most common pathogenesis is the straighteningand obstruction of the coronary arteries fromatherosclerosis.Nowadays,in developed and developingcountries ,IHD is a fetal and deforming disease with highmorbidity and severe harm, which imperils man’s health.In recent years, the IHD morbidity has been increasing andhas become the second leading cause of death in China. Themortality on cardio-vascular system diseases in urbanareas of China has increased over that of America andCanada, to make our country become a high occurrence placeof cardio-vascular disease. Therefore, it has become animportant topic in medical field to prevent and treat IHD.Presently, medication therapy is still the most importanttreatment against IHD. Drugs which can increase coronaryblood flow, oxygen supply and decrease myocardial oxygenconsumption are of manifold benefits to myocardialischemia.Dragon’s Blood (DB), also named Kylin’s Blood, is akind of red resin secreted by Dracaena. DB is a valuableTraditional Chinese Medicine, which was used to treatwounds and gynecological diseases in ancient times. DB hasbeen recorded and put into use for more than 1500 years,but mainly relied on imports for many years. Since 1972,with the discovery and development of Dracaena in YunNanand GuangXi province, the preclinical study and clinicalapplication has been greatly developed and the importingperiod has been terminated. Modern studies prove that DBhas the following pharmacological effects, includingtwo-direction adjusting blood rheology,, removing oxygenfree radicle,activating blood, inhibiting thrombosis,diminishing inflammation, easing pain, decreasing serumcholesterol, resisting arrhythmia and improve immunefunction. It is used to clinical medicine againstgastrointestinal hemorrhage, burn, extensive bedsores,colonitis, coronary heart disease, fracture and manykinds of gynecological diseases. Experiment animalstudies have discovered that DB can decrease bloodviscosity, plasma viscosity, hematocrit, increaseelectrophoresis velocity of red blood cell and platelet,inhibit platelet aggregation and thrombosis. DB has beenapplied to treat brain infarction and atherosclerosisheart disease, showing reliable effect. Little data hasbeen reported about the effects of DB on myocardialischemia in experimental animals. So that, myocardialischemic dogs model was used in our study to observe theeffects of hemodynamics, myocardial infarction area, EECGand enzyme activity, to explore the functional mechanismof DB powder extracted in a new method on anti-myocardialischemia. Simultaneously, the effects of three dosageswere compared. From this experiment the followingconclusions can be drawn:(1) The effect of DB related to dilating coronary artery,increase myocardial blood supply and decrease myocardialoxygen consumption. These illustrate DB can improvecardiovascular system function, which is beneficial toischemic heart disease.(2) The effect of DB on electrophysiology of myocardialinfarction dogs. EECG results show, DB could obviouslydecrease myocardial ischemia degree (∑-ST) and reducemyocardial ischemia coverage(N-ST,point), whichindicates DB has the effect of anti-myocardial ischemia.(3) The effect of DB on pathobiology of myocardialinfarction dogs. Experiment results show, DB couldobviously reduce infarction area (P<0.05,P<0.01). Themyocardium tissue section observed through lightmicroscope shows that DB could mitigate myocardialischemia of myocardial infarction dogs.(4) The effect of DB on AST, CPK, LDH, MDA and SOD ofmyocardial infarction dogs. Experiment results show, DBcould decrease the serum content of AST, CPK, LDH and MDA,and improve the activity of SOD. These results indicateDB could enhance the ability of removing free radicles,reduce the free radicle damage from hypoxia and protectthe cellular membrane. These show that DB could protectischemic myocyte.(5) The effects of different dosage DB on myocardialinfarction dogs were compared. Experiment results show,DB 120mg/kg is better than other two dosage teams. It hasthe best effect on anti-myocardial ischemia.Above results show that DB extracted in a new methodcould slow heart rate, decrease coronary resistance,
