自体造血干细胞移植治疗多发性硬化

【摘要】 目的:探讨多发性硬化(MS)患者骨髓造血干/祖细胞体外培养的增殖分化功能及其临床应用的意义。方法:研究对象为19 例MS 患者。设健康对照22 例。采用体外胶原半固体培养法观察骨髓造血干/祖细胞巨核系集落形成情况,甲基纤维素半固体培养法观察粒-单系和红系集落形成情况。各系与健康对照组分别进行比较,然后将19 例MS 患者按复发期和缓解期分组及按病程≤3 年和病程>3 年分组再进行比较分析。结果:(1) MS 患者巨核系细胞集落形成单位(CFU-MK)、粒-单系细胞集落形成单位(CFU-GM)、红系细胞集落形成单位(CFU-E)和健康对照组相比较,差异无统计学意义(均P>0.05)。(2) MS 复发期组的CFU-GM 显著低于健康对照组(p=0.006),也显著低于缓解期组(p=0.001)。MS 缓解期组CFU-GM 虽比健康对照组较高,但差异无统计学意义(p>0.05)。MS 复发期组、缓解期组的CFU-MK、CFU-E 与健康对照组相比,差异均无统计学意义(均p>0.05)。(3) MS 病程≤3 年组和病程>3 年组的CFU-MK、CFU-GM、CFU-E 与健康对照组相比,差异均无统计学意义(均p>0.05)。结论:提示MS 复发期患者骨髓造血干/祖细胞本身或其分化成粒-单系的过程存在异常;MS 复发期造血干/祖细胞也可能对造血生长因子的反应较缓解期差。
【Abstract】 Objective: To explore in vitro hematopoietic stem/progenitor cell growth and proliferation of bone marrow in multiple sclerosis (MS) patients and its clinical application in MS treatment when autologous peripheral blood stem cell transplantation (APBSCT) was performed. Methods: In vitro, clones and proliferation of CFU-MK in bone marrow hematopoietic stem/progenitor cells were identified with semisolid collagen, and those of CFU-GM and CFU-E with semisolid methycellulose in 19 MS patients and 22 healthy controls. Results: (1)There were no significant differences in CFU-MK、CFU-GM and CFU-E counts between general MS patients and healthy controls respectively(P>0.05).(2) The CFU-GM counts of MS patients in relapsing phase were lower than those of healthy controls(p=0.006)and they were lower than those of MS patients in remitting phase as well (p=0.001). There were no significant differences in CFU-MK and CFU-E counts among MS patients in relapsing or remitting phase and healthy controls(P>0.05). (3) There were no significant differences in CFU-MK、CFU-GM and CFU-E counts among MS patients whose disease course within or over 3 years and healthy controls(P>0.05). Conclusions: There were abnormal proliferation and differentiation of bone marrow hematopietic stem/progenitor cells in relapsing phase of MS. The hematopietic stem/progenitor cells might be less sensitive to hematopoietic growth factor in relapsing phase of MS than those in remitting phase of MS.
