【摘要】 目的:探讨不同方法治疗儿童肱骨髁上骨折的临床疗效。方法:采用三种不同治疗方法治疗70例肱骨髁上骨折,A组30例采用手法复位L型纸壳夹板外固定,B组25例采用闭合复位交叉穿针内固定,C组15例采用切开复位交叉克氏针内固定。观察骨折愈合情况、肘关节屈伸功能及肘内翻的发生率。结果:A、B组骨折愈合时间明显快于C组,有显著性差异,A、B组之间无显著性差异。关于肘关节屈伸功能A、B组明显优于C组,有显著性差异,A、B组之间无显著性差异。在肘内翻发生率方面B组优于A组,有显著性差异,C组稍优于B组,但无显著性差异。结论:对于无移位或轻度移位的稳定性肱骨髁上骨折,可采用手法整复外固定。McIntyreⅢ_a、Ⅲ_b型严重移位或旋转等不稳定性骨折,手法复位成功后经皮穿针疗效较好。具有手术指征者,切开复位交叉克氏针内固定,可明显降低肘内翻的发生。更多还原

【Abstract】 Objective: To investigate the effect of supracondylar fracture of humerus in children treated with different methods. Methods: 70 cases of supracondylar fracture of humerus were treated with 3 methods, group A was treated with manual reduction and splint external fixation made of cardboard. group B was treated with closed reduction and percutaneous crossed-pin interior fixation. group C was operated with crossed-pin and interior fixation. Results: About the time of fracture concrescence: In Ridit analysis, the difference of therapeutic effect was significant between group A、B and group C, but group A and group B has no statistical significance. About the function of the elbow: The result of statistical treatment demonstreated that the difference between group A、 B and group C has statistical significance, but group A and group B has no statistical significance. About the rate of occurrence of cubitus varus: group B excelled group A and has statistical significance, group C excelled group B a little, but group B and group C has no statistical significance. Concltsion: Stable fractures which have no displacement or little displacement can be treated with manual reduction and external fixation. Closed reduction and percutaneous crossed-pin fixation is preferred for the treatment of those unstable fractures such as McIntyre type III. For those needing operation, crossed-pin and interior fixation has very good effect with less possibility of cubitus varus.更多还原
