安宫牛黄散中朱砂、雄黄对HSP70、iNOS及细胞因子的影响

【摘要】 研究背景:安宫牛黄丸出自清代吴鞠通《温病条辨》,具有清热解毒、豁痰开窍的功效,主治热病、邪入心包、神昏谵语。安宫牛黄丸治疗急性脑血管病,疗效显著,后遗症相对较轻,是临床上治疗高热惊厥、脑卒中的要药。 朱砂化学成分主要为硫化汞(HgS),《药典》规定朱砂中硫化汞含量不得低于96%。朱砂具有镇惊、催眠、抗惊厥作用。雄黄主要成分为二硫化二砷(As2S2)。《药典》规定雄黄中二硫化二砷含量不得低于90%。雄黄具有抗菌、抗肿瘤作用。 目的:研究病理状态下(外伤性脑水肿),安宫牛黄散中朱砂、雄黄对HSP70、iNOS及细胞因子(TNF-α、IL-1β)的影响,探讨复方中朱砂、雄黄的药理作用机制,为该方的临床应用提供科学依据。 方法:本实验选用外伤性脑水肿动物模型。SD大鼠,雌雄各半,分为8小时组和24小时组,每组又分设6个组:正常组、模型组、模型+朱砂组、模型+雄黄组、模型+安宫牛黄散组(下称整方组)、模型+去朱砂、雄黄的安宫牛黄散组(下称拆方组)。RT-PCR方法测定脑组织中HSP70mRNA含量;化学法测定脑组织中NOS含量;放免法测定血清中细胞因子TNF-α、IL-1β。 结果:造模后8小时,安宫牛黄散整方组与拆方组HSP70mRNA表达均较模型组增高(P<0.05),且整方组诱导HSP70mRNA表达的作用强于拆方组(P<0.05)。 造模后8小时,朱砂组、雄黄组、安宫牛黄散整方组、安宫牛黄散拆方组iNOS表达均较模型组降低(P<0.05)。整方组抑制作用最为显著,与朱砂组、雄黄组及拆方组比较,差异有显著性意义(P<0.05)。 造模后8小时,雄黄组、整方组、拆方组TNF-α表达较模型组降低(P<0.05)。但整方组与雄黄组、拆方组比较,差异无显著意义(P>0.05)。整方组与拆方组IL-1β表达较模型组降低(P<0.05),但两者之间的差异无显著意义(P>0.05)。 造模后24小时,各组HSP70mRNA、iNOS、TNF-α及IL-1β表达与正常组比较无显著差异。 结论:正常大鼠脑组织中仅有少量HSP70mRNA表达,造模后8小时,HSP70mRNA显著增高;造模后24小时,恢复正常水平。正常大鼠脑组织中iNOS含量极低,脑外伤后8小时,iNOS含量显著增高;24小时,降至基础水平。正常大鼠血清中细胞因子仅有较低水平表达;脑外伤后,TNF-α、IL-1β表达显著增加,伤后8小时仍维持较高水平的表达,外伤后24小时,恢复基础水平。

【Abstract】 BACKGROUNDS: Angong niuhuang pill is the most famous medicine for emergency in Chinese traditional medicine .It is a fragrant drug for disspating heat, detoxifying, tranquilizing and resuscitation. It has a wide application in clinic practice on stricken by wind.The basis of cinnabaris is mercuric sulfide (HgS) . < Chinese pharmacopoeia > prescribe mercuric sulfide is not less than 96% .It has effect on tranquilization , hypnogenesis and therapy of epileptitrom convulsion .The basis of realgar is arsenic disulphide (AS2S2). < Chinese pharmacopoeia > prescribe arsenic disulphide is not less than 90% . It is antibacterial anticancer.OBJECTIVE: To study the effect of Angong niuhuang pulvis on HSP70,iNOS and CKs on pathological body and investigate initially the effect of cinnabaris and realgar therein providing scientific foundation for application in clinic practice.METHODS: Sprague-dawley rats including male and female were statistics randomly divided into two groups for 8 hours’and 24 hours’ group . Both of them were divided in to 6 groups : normal control group , cerebral injury model group ,cerebral injury rats administrating cinnabaris , cerebral injury rats administrating realgar , cerebral injury rats administrating Angong niuhuang pulvis ,cerebral injury rats administrating Angong niuhuang pulvis excluding cinnabaris and realgar . At various times , the brain of rats were removed just after blood serum taken . The amount of HSP70mRNA ,iNOS in the rats’ brain were estimated . So did the amount of CKs in the rat blood serum .RESULTS; The activities of HSP70mRNA of rat’s brain in the groups administrating Angong niuhuang pulvis and Angong niuhuang pulvis excluding cinnabaris and realgar increase obviously compared with the cerebral injury model group 8 hours after injury (P<0.05) .The activity of HSP70mRNA in the group administrating Angong niuhuang pulvis is higher than that in the group administrating Angoug niuhuang pulvis excluding cinnabaris and realgar (P<0.05) .The activities of iNOS of rat’s brain in the groups administrating cinnabaris, realgar, Angong niuhuang pulvis and Angong niuhuang pulvis excluding cinnabaris and realgar decrease obviously compared with the cerebral injury model group 8 hours after injury (P <0.05) .The activity of iNOS in the group administrating Angong niuhuang pulvis is lower than that in the groups administrating cinnabaris , realgar and Angong niuhuang pulvis excluding cinnabaris and realgar (P<0.05) .The activities of TNF- a of rat’s serum in the groups administrating realgar, Angong niuhuang pulvis and Angong niuhuang pulvis excluding cinnabaris and realgar decrease obviously compared with the cerebral injury model group 8 hours after injury (P<0.05) . However, there were no significant difference among the activities of TNF- a in the groups administrating realgar , Angong niuhuang pulvis and Angong niuhuang pulvis excluding cinnabaris and realgar . The activities of IL-1 ^ of rat’s serum in the groups administrating Angong niuhuang pulvis and Angong niuhuang pulvis excluding cinnabaris and realgar decrease obviously compared with the cerebral injury model group 8 hours after injury (P <0.05 ) .But the activities of IL-1 (3 in the serum have no significant difference between the the group of Angong niuhuang pulvis and that of Angong niuhuang pulvis excluding cinnabaris and realgar.The activities of serum and brain HSP70mRNA, iNOSJNF- a and IL-1 3 have no significant difference between the 24 hours’ groups and the normal groups .CONCLUSION: The amount of HSP70mRNA is fewer in the normal rat’s brain .It incresses obviously 8 hours after cerebral injury ; it comes back 24 hours after cerebral injury . The amount of iNOS is fewer fewer in the normal rat’s brain .It incresses obviously 8 hours after cerebral injury ; it comes back 24 hours after cerebral injury . The amount of CKs is fewer fewer in the normal rat’s brain .It incresses obviously 8 hours after cerebral injury ; it comes back 24 hours after cerebral injury.The activities of HSP70mRNA of rat’s brain in the groups administrating Angong niuhuang pulvis and Angong niuhuang pulvis excluding cinnabaris and realgar increase obviously compared with the cerebral injury model group 8 hours after injury (P< 0.05) .The activity of in the group administrating Angong niuhuang pulvis is higher than that in the group administrating Angong niuhuang pulvis excluding cinnabaris and realgar (P < 0.05 ) . Neither cinnabaris nor realgar could induce HSP70mRNA by itself.
