中药配合穴位按摩治疗糖尿病性周围神经病变的临床观察

【摘要】 本论文主要包括文献综述和临床研究两个部分。 文献综述共两篇。第一篇回顾了近年来国内外对本病的基础和临床研究进展。主要从机理研究和治疗方面进行了论述。对于发病机理,至今仍未阐明清楚。目前临床治疗药物很多,但总体疗效,尤其是严重的患者疗效并不理想。第二篇着重从中医辨证治疗、专方治疗、中成药治疗、针灸治疗、中药泡洗治疗等方面概述近 5 年来中医药治疗 DPN的进展。 临床研究部分观察了中药糖络宁配合穴位按摩对糖尿病性周围神经病变(DPN)的临床疗效。将符合标准的 DPN 病人 60 例,随机分为治疗组(30 例)和对照组(30 例),在基础治疗的同时,治疗组予具有益气养阴、滋补肝肾、活血通络的糖络宁治疗,同时配合局部穴位按摩治疗;对照组予口服弥可保(甲钴胺)治疗。对病人的症状、神经传导速度及波幅进行了观察。结果:治疗组的总有效率为 93.33%,对照组的总有效率为 63.3%。经 Ridit 检验显示治疗组明显优于对照组(P<0.05)。结果还显示糖络宁配合穴位按摩对DPN 病人的肢体麻木、疼痛、触觉、针刺觉、震动觉、踝反射减弱等神经系统症状改善明显,对神经传导速度及波幅有明显的改善作用。治疗过程中未见明显的不良反应。结论:糖络宁与穴位按摩合用可明显提高 DPN 临床疗效,提示中医综合治疗 DPN 有着明显优势。更多还原

【Abstract】 The paper consists two parts: the summaries of documents and a clinic research. There are 2 sumaries of documents. The first one is about the research progress of the disease in recent years. It includes the cause research and treatment method.The cause of diabetic peripheral neurophathy(DPN) has not clearly expounded yet today.There are many medicine used in clinic, but the curative effect is not so good,especially in the serious patients.The second one introduces the treatment progress of TCM in DPN in recent 5 years. It includes the treatments applying with the pattern identification,the treatments of herbs, the treatment of Chinese patent medicine,the treatment of acupuncture , herbs immersing and so on. The clinical part is about the research of efficacy for treating diabetic peripheral neuropathy DPN) with Tangluoning and points massage 60 DPN patients conforming to the diagnostic criteria were randomly divided into the herb group(30 patients)and control group(30 patients).On the basis of basis treatment,the herb group was given Tangluoning(for benefiting qi and nourishing yin, tonifying the liver and kidney,and activating blood circulation) and points massage.The control group is given the methycobal(mecobalamin).The changes in the symptoms, nerve conductive velocity and amplitude were observed in the patients.The results showed that the total effective rate of the herb group was 93.33% and that of control group was 63.3% . By the Ridit test,the effect of herb group was better than the control group, the difference was very significant(P<0.01).And The results also showed that the herb group and the control group had obviously difference in symptoms of the limbs,pain, weakened ankle reflex,improveing the nerve conductive velocity and amplitude.There were no obvious side effects found during the treatment.The study showed that Tangluoning and points massage had significant effect for the DNP,better than methycobal.更多还原
