股骨髁髌股、胫股关节重叠接触区域及其生物生物力学测定

【摘要】 股骨髁髌股、胫股关节运动重叠接触区域及其生物力学测定 目的 明确膝关节运动过程中胫股、髌股关节在股骨髁关节面接触的重叠区域,测量并比较、分析在此区域内各自接触压大小及其分布特点,探讨髌股关节置换术中股骨滑车假体长度的设计。方法 Instron 8501生物力学测试仪上模拟生理状态下膝关节站立到下蹲屈膝全过程,收集8具新鲜膝关节尸体标本,通过染色法分别描记髌股、胫股关节运动在股骨髁面接触区域,标记两者在股骨髁关节面运动的重叠接触区域区域;利用富士公司超低敏压敏片分别测量重叠接触区域内髌股、胫股关节在该区域接触压;结果 髌股、胫股关节在股骨髁关节面运动时存在重叠接触区域,范围大致相当于髌股关节在股骨髁面接触区域内始于屈膝75°髌股接触区的后部分接触区域;重叠区域内髌股、胫股关节各自接触压测定发现,髌股关节不同区域接触压无显著性差异,但其接触压明显高于胫股关节;结论 髌股关节置换术中的股骨滑车假体置换的范围应该涵盖髌股、胫股关节在股骨髁运动的重叠接触区域,即股骨滑车假体的有效长度应涵盖髌股关节在股骨髁关节面所有接触区域。

【Abstract】 Objective: Ascertain the common area on the femoral condyle facet which the patellofemoral and tibiofemoral joint moving , and determine their pressure on this area in order to searching for the efficient length of the femoral trochler prosthesis of the PFA. Method: 8 fresh cadaver knees were mounted on Instron 8501 so that life-like force of weight-bearing were simulated from standing to high flexion. Find out the common area through drawing up their contact areas on the femoral condyle which the patellofemoral and tibiofemoral joint moving by the dye method, and test their contact pressures of the area with Fuji pressure film. Result: There is a common contact area on the femoral facet which is the within the patellofemoral contact area beginning 75 flexion. The tibiofemoral contact pressure is larger than that of patellofemoral joint on this area. Conclusion: The femoral trocher prosthesis would contain the whole contact area of patellofemoral joint.
