软骨素酶联合纤维蛋白溶解酶诱导玻璃体后脱离的实验研究

【摘要】 [目的] 通过动物实验,确定软骨素酶以及软骨素酶联合纤维蛋白溶解酶在兔眼中产生玻璃体后脱离的时间和程度,并评价其安全性。 [方法] 将18只白兔随机分为3组,每组均为6只白兔。右眼为实验眼,左眼为对照眼。玻璃体腔分别注射不同药物:第1组实验眼注射软骨素酶ABC 0.1U/0.1ml;第2组实验眼注射软骨素酶ABC 0.1U/0.05ml和纤维蛋白溶解酶(Roche公司)0.5U/0.05ml;第3组实验眼注射软骨素酶ABC 0.1U/0.05ml和纤维蛋白溶解酶(Calbiochem公司)1.0U/0.05ml;对照眼均注射0.9%生理盐水0.1ml。在注射酶类后2小时、1天和1周时,分别进行裂隙灯加+78D前置镜、视网膜电生理检查和B超检查,每次检查后随机取2只白兔,以空气注射法处死,剜出眼球,标本分别做光学显微镜和透射电镜检查。 [结果] B超显示第2组实验眼在2小时发现5例部分性玻璃体后脱离,在1天时出现4例部分性玻璃体后脱离,在1周时出现1例部分性玻璃体后脱离及1例完全性玻璃体后脱离;第3组实验眼在2小时发现4例部分性玻璃体后脱离,在1天时出现3例部分性玻璃体后脱离,在1周时出现1例部分性玻璃体后脱离及1例完全性玻璃体后脱离;第1组实验眼和各组对照组在各个时间段均没有发现玻璃体后脱离。除第2组实验眼ERG检查表现为不可逆性降低以及病理学明显毒性改变外,其余各组均ERG检查与对照组无明显差异,病理学检查无特异性改变。 [结论] 1.0.1U软骨素酶单独眼内注射在1周内不能产生玻璃体后脱离。2.0.1U软骨素酶联合0.5U或1.0U纤维蛋白溶解酶眼内注射可以在短期内产生玻璃体后脱离,但在1周内产生的玻璃体后脱离为部分性。3.0.1U软骨素酶联合0.5U纤维蛋白溶解酶(Roche公司)眼内注射产生严重的眼内毒性,明显影响眼的视觉功能,而0.1U软骨素酶联合1.0U纤维蛋白溶解酶(Calbiochem公司)眼内注射是安全的,对视网膜功能无明显影响。

【Abstract】 [Objective] To investigate the toxicity of chondroitinase ABC and plasmin, and evaluate their efficacy in the production of posterior vitreous detachment. [Method] 18 young Newsland rabbits were randomized into 3 groups, 6 rabbits in each group. Different enzymes were injected intravitreal in right eyes: group 1 received chondroitinase ABC 0.1U/0.1ml; group 2 received chondroitinase ABC 0.1U/0.05ml and plasmin (Roche) 0.5U/0.05ml; group 2 received chondroitinase ABC 0.1U/0.05ml and plasmin (Calbiochem) 1.0U/0.05ml, all left eye received intravitreal 0.9% NS 0.1ml as control.Slit-lamp biomicroscopy ,+78 diopter lens, Electroretinogrphy and ultrasonograph examination were performed at 2 hours, 1 day and 1 week after injection. 2 rabbits were killed after each examination and the eyes were enucleated. Light and Eletron microscopy were performed to study the vitreretinal interface.[Result] In group 2, 5 partly PVD were found at 2 hours after injection, 4 partly PVD were found at 1day, 1 partly PVD and 1 complete PVD were found at lweek in the right eyes; In group 3, 4 partly PVD were found at 2 hours, 3 partly PVD were found at lday, 1 partly PVD and 1 completely PVD were found at lweek in the right eyes. No PVD were found at any time in group 1 and the left eyes, a, b wave in ERG display irreversible decrease and severe toxicity to retina in histopathological examination in the right eyes of group 2., But no change were found in the other groups and the control eyes in ERG and histopathological examination. [Conclusion] 1. Posterior vitreous detachment can not induced by 0.1U chondroitinase ABC intravitreal alone in 1 week in rabbit eye. 2. Posterior vitreous detachment can induced by 0.1 U chondroitinase ABC combined with 0.5U or 1.0U plasmin in 1 week in rabbit eye, but the PVD is partly in 1 week. 3. 0.1U chondroitinase ABC combined with 0.5U plasmin (Roche) intravitreal injectioncould cause severe toxicity to retina and affect the visual acuity;and 0.1U chondroitinase ABC combined with 0.5U plasmin(Calbiochem) intravitreal injection is safe without any harm to retina.
