肠内生态营养对严重腹腔感染大鼠肠粘膜屏障及血浆IL-6、TNF和内毒素的影响

【摘要】 外科病人的营养支持已成为现代外科治疗的重要组成部分,被认为是20世纪后叶医学的重大进展。60年代后期迅速发展起来的全肠外营养,已提供机体所需要的已知营养素。80年代中期,肠道细菌易位所导致的代谢偏离生理过程,代谢并发症增加。90年代以来,在病患得到初步处理后,考虑给予营养支持,以防早期缺乏营养底物而加重创伤导致的细胞代谢障碍,Saito等[3]发现,肠道早期营养,回肠和空肠粘膜重量显著于早期静脉营养,此外,可显著增加血清胃泌素水平,促进胃肠粘膜生长及运动,清除肠道内容物淤滞,防止细菌与粘膜长期接触损伤肠粘膜。近年对营养支持的一个新认识就是认为EN优于PN,只要条件允许应尽可能采用EN,肠内营养的主要优点有:促进肠内蛋白合成,增强免疫功能,减轻创伤应激反应,促进肠道激素与免疫球蛋白释放,改善肠粘膜的渗透性,维护肠粘膜屏障功能,减少肠道细菌内毒素易位。有研究表明只要提供不低于总热量20%的肠内营养就可避免肠道屏障功能的破坏与肠道德细菌移位,在重危感染病人肠内营养的药理作用大于其营养作用。 肠内生态营养对严重腹腔感染大鼠肠粘膜屏障及血清IL-6、TNF和内毒素的影响。 本实验中,我们将40只SD大鼠随机分成:假手术组、手术对照组、肠内营养组和添加生态制剂的肠内营养组,每组10只,通过大鼠盲肠结扎穿孔制造腹腔感染大鼠动物模型通过盲肠结扎穿孔法制作腹腔感染大鼠模型,5天后宰杀动物,通过光镜观察肠粘膜形态,<WP=31>在不同时相检测大鼠死亡情况并检测血液生化指标IL-6、TNF、及内毒素水平。结果表明:添加生态制剂的肠内营养组IL-6,TNF、内毒素较手术对照组下降明显,肠粘膜形态完整,脓毒症状表现减轻,死亡率降低。常规病理观察见:假手术组大鼠除肠粘膜下层可见少量淋巴细胞浸润外,未见明显形态学改变;空肠及回肠肠粘膜上皮完整,绒毛排列整齐,粘膜、粘膜下层及肌层各层次清晰。Clp组大鼠肠粘膜充血明显,间质小血管扩张,可见大量中性粒细胞浸润,间质水肿;肠管扩张,绒毛变矮、畸形,局灶性绒毛上皮脱落常见;随着感染加重,粘膜上皮坏死脱落加重,部分固有层裸露,固有层腺体明显减少。EN组肠粘膜可见明显炎症反应,大片绒毛坏死脱落少见,小肠绒毛形态较正常。添加生态制剂的EN组大鼠小肠粘膜基本完整,部分绒毛变钝,固有层腺体丰富。随着营养支持的持续,粘膜变薄,绒毛缩短,部分绒毛可见间质水肿,固有层有少量炎性细胞浸润。血清TNF、IL-6、内毒素的水平在模型制成后6小时开始升高,CLP血清TNF较假手术组明显升高24~48小时达高峰,之后略有下降,但仍高于6小时;EN组和E组各点均较CLP组明显降低,但仍高于假手术组;E组各时相点亦均低于EN组(p<0.01)。 90年代以来的研究表明,营养支持可以改变疾病的治疗效果,不仅是由于纠正病人的营养不足,更重要的是通过其中特异营养素的药理学作用达到治疗目的。在严重腹腔感染、严重创伤、手术、放化疗、重症胰腺炎以及长期肠外营养情况下,有时发生抗生素难以控制的感染。在应激状态肠道粘膜的结构和功能可能受到严重损害,造成细菌移位,目前被认为是继发性肠源性感染的主要原因。已经证实肠<WP=32>内营养可增加内脏的血流量,给胃肠粘膜提供营养素,促进胃肠激素得分促进胃肠蠕动,保护胃肠道;而生态营养主要通过调整肠道微生态、对致病菌的拮抗作用,中和肠道内有毒物质、增强肠道和全身免疫功能、提高营养素消化吸收率等作用来改善肠道内环境,最终达到维护肠道生态环境稳定。生态免疫营养能恢复肠道功能改善机体营养和免疫状态,减少危重病人的感染率,降低死亡率。小肠粘膜细胞代谢活跃、更新迅速,粘膜细胞半衰期大鼠为4~5天,人为6~7天。小肠绒毛中老化凋亡的细胞由隐窝腺凹产生的新生细胞补充修复,并由终末动脉血管分支供血,因而易受缺血、缺氧损害。长期禁食后因缺乏食物刺激,消化液和肠道内分泌激素分泌减少;创伤、感染等应激后并发缺血再灌注损伤,导致肠绒毛血供下降。此外,在炎症介质的释放等因素的共同作用下,可能导致肠粘膜损害。手术创伤、烧伤和严重感染后的动物和临床研究均已观察到肠粘膜损伤改变的明确结果。本研究中Clp组大鼠肠粘膜充血明显,间质小血管扩张,可见大量中性粒细胞浸润,间质水肿;肠管扩张,绒毛变、畸形,局灶性绒毛上皮脱落常见;随着感染加重,粘膜上皮坏死脱落加重,部分固有层裸露,固有层腺体明显减少。EN组肠粘膜可见明显炎症反应,大片绒毛坏死脱落少见,小肠绒毛形态较正常。EN和感染后大鼠小肠粘膜均表现萎缩趋势,在毒素的作用下这种趋势更为明显。肠粘膜正常结构和功能的维持是肠道正常吸收和屏障功能的基础,肠粘膜结构和功能的改变导致肠道粘膜通透性增加,肠道细菌或内毒素移位,成为机体应激状态持续或发展成多器官功能障碍的基础,在应用生态制剂后,大鼠小肠粘膜基本完整,部分绒毛变钝,固有层腺体丰富。随<WP=33>着营养支持的持续,粘膜变薄,绒毛缩短,部分绒毛可见间质水肿,固有层有少量炎性细胞浸润。我们考虑与双歧杆菌参与有关。研究表明,双歧杆菌、乳酸菌和粪链球菌能在肠粘膜表面与粘膜

【Abstract】 The nourishment support of the surgical operation patient has become the importance parts of the modern surgical operation treatment, being seen as important advanced of medical science in the twenty centuries. The nutriment of EN has the main parts of benefits in the intestine, such as promoting the protein synthesizes, enhancing the immunity function, alleviating the damage of the stress response, promoting the releases of intestine hormone and immune globulin, improving intestinal microcirculation and reducing the severity of intestinal muscosal injury. Some investigation indicated that total calories 20% of enteric nutrition could avoid the injury of gut barrier faction and intestinal bacterial translocation. The function of medicine is more weight than its nutrition in some serious infection patients. The aim of the study was to investigate the effect of enteral ecoimmunonutrtion on gut barrir, serum TNF, IL-6 and endotoxin in serious intra-abdominal of the rats In this experiment, we use 40 SD rats that are divided into four groups: (1) artificial operation groups (n=10), (2) cecal ligation and puntures (CLP) groups (n=10), Enteric Nutrition (EN) groups (n=10), (4) Econonutriyion groups (n=10). The suspension was exposed to the environment for two hours in this experiment, all animals were killed after five days and <WP=35>were examined the tissue of intestine, at the same time, examined the serum TNF, IL-6 and endotoxin in different time during the study. Passing optical microscope observe, we found: The intestine muscoma instruction of artificial operation groups big rats had few changing. Others had different injury, especially the CLP groups were serious. The intestine muscoma instruction of EN groups was not serious compared with the CLP groups. The level of the serum TNF, IL-6, endotoxin is start going up after 6 hours the model being made. The serum TNF level of artificial operation and CLP groups goes up obviously during 24 hours, and the 48 hours reach the high peak, and then having the descent slightly The serum TNF level of the enteric nutrition (EN) groups is lower than that of the CLP groups obviously, but still higher than that of artificial operation groups; Each hour of the Econonutriyion groups orders mutually as well all lower than the set of EN. (p<0.01). Our study show that the Econonutriyion can cut down the level of the serum TNF, IL-6, endotoxin, moderate the poisonous material in an inside intestine, adjust intestine for tiny ecosystem, ameliorate intestine inside environment and lowering the infection rate and death rate.
