头穴丛刺法对急性缺血性脑梗死大鼠神经可塑性影响的研究

【摘要】 目的:进一步阐述头穴丛刺法对急性脑梗死治疗作用的机制。方法:运用血管内栓线法制备wistar大鼠局灶性脑缺血的动物模型,模拟人类急性脑缺血损伤的病理变化,以神经功能评分,病理形态学,电镜观察梗死灶边缘区神经元结构,突触的数目、形态,突触后致密物质为指标,进行了术后24h、7d、21d各组间及组内的对比观察。结果:(1)治疗前造模组与头穴丛刺治疗大鼠神经功能评分,无明显差异,21天头穴丛刺治疗能够提高脑梗死大鼠平衡、行走及抓握能力改善患侧肢体功能;(2)24h造模组与24h头穴丛刺治疗组光镜下无明显差别,脑组织坏死灶形成,周围血管充血出血;7天头穴丛刺治疗组光镜下可见梗死灶周围新生血管增多、胶质细胞增生,优于对照组;21天头穴丛刺治疗组光镜下可见梗死灶周围小血管明显增生、胶质纤维增生,明显优于对照组;(3)抑制脑缺血,保护神经元突触结构的完整性,增加神经毡区突触数目、增强突触前后膜信号的传递,即突触活性区长度增加,突触后致密物质增厚,体现了突触的可塑性,促进了脑缺血损伤后功能环路的重建。结论:头穴丛刺能减少脑缺血性损伤所造成的细胞死亡,增强突触联系,对神经元具有保护作用,因而能提高神经功能,促进肢体功能康复,为该治疗方法在临床应用中提供了理论依据。

【Abstract】 In this subject, the male rats of Wistar strain were selected as experimental study objects. 40 rats were divided into four groups by random: false operation group (group A), model group (group B), Baihui to Qubin group (group C), cluster needling of scalp point group (D). To analogue the pathologic change coursed by cerebral ischemic in human, the animal model were made by blocking middle cerebral artery.Neuronal function evaluation, halter test, rotation test, reticular screen test, pathomorphologic charge, and cell apoptosis were selected as indexes. We contrasted the difference among different groups at 24 hours, 7 days and 21 days after the operation and tried to contrast observe the mechanism of cluster needling of scalp point therapy in acute cerebral infarction from the scopes of behavior medicine pathomorphology.The study showed that cluster needling of scalp point therapy can improve cerebral infarct rat’s brasping equilibrating, moving function and strengthen myodynamia; improve glia cell and collagenons fibers hyperplasia and reduce cerebral tissue edema and infarct volume. Protect the basic structure of neural synapse and lighten the infarction,increase the number of synapse, the length of synaptic active region (LSAR), thickness postsynaptic density (TPSD).The study proved that the therapy of cluster needling of scalp point lighten cerebral infarction. It can protect and repair neuron promote rehabilitation. This experiment provided theoretical basis to clinic.
