益气活血补肾法治疗糖尿病肾病的临床研究

【摘要】 目的 糖尿病肾病是糖尿病常见而难治的微血管并发症，已成为糖尿病病人的主要死因之一。迄今尚无有效方法能够防止其发展和恶化。如何有效的防治糖尿病肾病，已成为当前糖尿病和肾脏病学者们共同的研究热点。本研究的目的是为了观察和验证导师以中医益气活血补肾法为主，中西医结合治疗糖尿病肾病的疗效，为降低糖尿病患者微血管病变的发生率及死亡率提供新的思路及治疗方法。 设计 自身前后对照研究。 方法 选取符合条件的门诊和住院患者30例，按照患者病情分为代偿期和失代偿期，采用不同的治疗方法治疗。以一个月为一个疗程，治疗3个月后进行随访。观察患者治疗前后中医症候，血糖，尿蛋白，肾功等实验室指标水平。 结果 经过3个月加用中药的中西医结合治疗，患者自身症状，尤其是多尿，疲乏无力改善较明显，上述实验室指标，其降血糖的效果有显著性差异(P＜0.01)，患者尿微量白蛋白及尿常规，24小时尿蛋白定量有显著性差异(P＜0.01)血肌酐也明显改善(P＜0.05) 结论 以益气活血补肾为主的中西医结合治疗糖尿病肾病，可使患者尿蛋白排出减少，血清肌酐下降，提示此法是多方面，多途径的发挥其对糖尿病肾病的治疗作用。更多还原

【Abstract】 Objective Diabetic nephropathy (DN), one of the common and severe complication of micro-blood vessel disease, becomes the most died reason for the diabetic patients. By now we have not effective ways to prevent its development and deterioration How to prevent the DN effectively is the research hotspot by the diabetic and kidney scholars together. This research was to observe and confirm the effect on DN mainly by the method of tonifing QI; promoting blood flow and tonifying kidney and by combining Modern Medicine at the same time .In order to decrease the occurrence rate and death rate of the micro-blood vessel disease, the research can offer a new idea and treatment. Project Self-contrast before and after treatment Methods Thirty patients (in-patient and out-patient) were chosen by certain standards and classified into compensation period and decompensation period due to different state of illness and treatment with different methods separately. Taken one month as a period of treatment and then there will be a visit after three months. The clinical manifestations, the glucose ,urine protein and the function of kidney of the patients were all observed by the method of self-contrast before and after the treatment.Result The syndromes of the patients especially at the long urine and fatigue and weak are more recovered than before treatment after three months by the combination of TCM and Modern Medicine. The effect of decreasing glucose has a remarkable difference (p<0.01) at the sametime the Microalbuminuria , urine routine test, the ratio of urine protein for 24 hours creatinine also have the remarkable difference (p<0.01). Conclusion The way that treating DN mainly by the method of tonifying QI, promoting blood flow and tonigying kidney and combing Modern Medicine can make the urine protein and creatinine decreased.
