软坚护肝片对肝纤维化影响的实验研究

【摘要】 目的:探讨软坚护肝片对实验性肝纤维化大鼠的影响 方法:应用40%四氯化碳和酒精联合建立肝纤维化大鼠模型。第一阶段(预防肝纤维化实验),取清洁级雌性Wistar大鼠38只,随机分成三组,①正常组8只②未干预组15只③软坚护肝片干预组15只。未干预组、干预组按40%四氯化碳皮下注射制备肝纤维化模型,用花生油将四氯化碳稀释成40%的溶液,首次按体重5ml/kg皮下注射,以后3ml/kg注射,三天一次,正常组以0.9%NaCl溶液2ml/只代替。在造模的同时软坚护肝片干预组给以软坚护肝片蒸馏水水溶液1.55g/kg.d,相当于生药量30.107g/kg.d,灌胃,5次/周。正常组与未干预组给以同等剂量的生理盐水灌胃。未干预组和软坚护肝片干预组长期饮用20%酒精。六周后未干预组大鼠、正常组和干预组大鼠,麻醉后腹腔内动脉采血,离心3000转/分,15分,取血清,留置—80℃冰箱保存,测透明质酸(HA)、Ⅳ型胶原(ⅣC)、层粘连蛋白(LN);切取肝叶组织放入10%中性磷酸福尔马林溶液中固定,进行病理切片,做HE染色,进行纤维化分期;胶原纤维Masson’s染色、VG染色,病理图像分析系统测定胶原纤维总面积和百分比。第二阶段(治疗肝纤维化实验),大鼠58只,按上述方法制造肝纤维化模型,造模成功的肝纤维化大鼠再随机分成三组:空白对照组、秋水仙碱组和软坚护肝片治疗组,治疗组给以软坚护肝片水溶剂灌胃,剂量同上,空白对照组以同等剂量的生理盐水做对照,秋水仙碱组给以秋水仙碱灌胃1.5mg/kg,5次/周。6周后按上述方式留取血清和肝组织做相同的指标测定。了解软坚护肝片对实验性肝纤维化大鼠的预防和治疗作用。 结果:①第一阶段(预防肝纤维化实验):正常对照组及干预组与未干预组比较,血清透明质酸(HA)、层粘连蛋白(LN)、Ⅳ型胶原(ⅣC)和纤维增生程度、胶原纤维面积及所占百分比减少。②第二阶段(治疗肝纤维化实验):治疗组和秋水仙碱组血清透明质酸(HA)、层粘连蛋白(LN)、Ⅳ型胶原(ⅣC)和胶原纤维增生程度、胶原纤维面积及所占百 分比较同期空白对照组明显下降,治疗组和秋水仙碱组无差异。结论: 1.软坚护肝片对大鼠肝纤维化形成有很好的预防作用 2.软坚护肝片可减少实验性肝纤维化大鼠肝脏胶原纤维的面积和降低血 清透明质酸、层粘连蛋白、IV型胶原水平。
【Abstract】 Objective: To probe the effect of RuJianHuGanPian on experimental hepatic flbrosis rats.Method: Establish the rat model of hepatic flbrosis by using 40 percent carbon tetrachloride and alcohol. First part (prophylactic experiment): 38 female Wister rats were randomly divided into three groups : normal group(n=8) , non-preventive group(n=15) , preventive group(n=15). Forty percent carbon tetrachloride (CC14) dissolved in peanut oil was injected subcutaneously to rats in non-preventive group and preventive group ( First 5 ml/kg , then 3ml/kg ,once threedays) for 6 weeks to produce hepatic flbrosis model . Normal group received Same dose Saline as a control .At the same time, preventive group rats were ingested with solution of RuJianHuGanPian (1.55g/kg daily ,5 times a week ),while those of normal group and non-preventive group only received same dose saline . 20% alcohol was administered to the non-preventive group and preventive group for a long time. After 6 weeks, all rats were killed. Serum hyaluronic acid, laminin , IVcollagen level and semi-quantitative histopathological area , percentage of collagen fibril and the stage of fibrosis were compared in each group .Second part (therapic experiment): 58 rats were divided into: positive control group, RuJianHuGanPian therapic group, Colchicine group. All rats were deduced into hepatic fibrosis model by using same method as at first part. After model had been established, saline, RuJianHuGanPian, Colchicine(1.5mg/kg 5 times a week )was administered respectively .At the end of the experiment, same indices were analysed . -Result: Experimental research: (1) First part (prophylacticexperiment ): Compared with non-preventive group , Serum hyaluronic acid, laminin , IVcollagen level and stage of fibrosis were lower in normal group and preventive group which total area , percentage of collagen fibril were decreased .(2)Second part (therapic experiment): The level of all parameters was higher in positive control group than that of RuJianHuGanPian therapic group and Colchicine group. There was not significant difference between RuJianHuGanPian therapic group and Colchicine group.Conclusion: 1. RuJianHuGanPian is effective in protecting against experimental CC14 induced liver fibrosis rats.2. RuJianHuGanPian can decrease the level of serum hyaluronic acid, laminin , IVcollagen and semi-quantitative histopathological area, percentage of collagen fibril in experimental CC14 induced liver fibrosis rats.
